This refers to
a special
testing
programs
team inspection
conducted
at the
Palo Verde Nuclear Generating Station, Units 1, 2,
and 3,
by Hr.
P.
H. Johnson
and
a team of NRC and contract inspectors
from April 25 through
Hay 27,
1993.
At the conclusion of the inspection,
the findings were discussed
with you and
those
members of your staff identified in the enclosed report.
As identified in the report, this inspection
examined the various types of
testing
conducted at your Palo Verde facility, including post-maintenance
testing,
post-modification testing,
Technical Specification surveillance
testing,
ASHE inservice testing, predictive maintenance
testing,
and other
specialized tests.
Within these
areas,
the inspection consisted of selective
examinations of procedures
and records,
interviews with personnel,
and
observation of activities in progress.
An executive
summary for this inspection is included in the enclosed
inspection report.
Strengths
noted
by the team included the definition of
your testing
programs,
the conduct of recent self-assessments,
and your
staff's thoroughness
in identifying and documenting technical
issues.
Weaknesses
identified by the team, principally involving the implementation of
your defined programs,
included the use of inappropriate test
acceptance
criteria, the resolution of identified technical
issues,
screening of proposed
design
changes,
and instances
of weak inter-organizational
communications.
Based
on the results of this inspection,
certain licensed activities appeared
to be in violation of NRC requirements,
as specified in the enclosed
Notice
of Violation (Notice).
These violations generally indicated
a need for
additional attention to the implementation of your existing program
requirements.

specified in the enclosed
Notice when preparing your response.
In your
response,
you should document the specific actions
taken
and any additional
actions
you plan to prevent recurrence.
After reviewing your response
to this
Notice, including your proposed corrective actions
and the results of future
inspections,
the
NRC will determine whether further
NRC enforcement
action is
necessary
to ensure
compliance with NRC regulatory requirements.
The team also identified two other violations,
as discussed
in Sections
2.2.2
and 7.3. 1. I of the enc'losed
inspection report.
These violations are not being
cited,
since appropriate
actions
had
been taken
by your staff,
and the other
criteria of paragraph
VII.B.1 of the
NRC Enforcement Policy were satisfied.
In accordance
with 10 CFR 2.790 of the NRC's "Rules of Practice,"
a copy of
this letter
and its enclosures will be placed in tt n
NRC Public Document
Room.
