Palo Verde Unit 1, 2, and 3 NOTICE OF VIOLATION Docket Nos. 50-528, 50-529, and 50-530 License Nos.
NPF-41, NPF-51, and NPF-74 During an NRC inspection conducted on Hay 31 through July 18, 1992, violations of NRC requirements were identified.
In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Action," 10 CFR Part 2, Appendix C, the violations are listed below:
A.
Unit 1 Technical Specification 6.8.1 states in part:
"Written procedures shall be established, implemented, and maintained covering the activities
. recommended in Appendix A of Regulator>
Guide 1.33, Revision 2, February, 1978."
Regulatory Guide 1.33, Revision 2, Appendix A, recommends procedures for performing maintenance of safety-related equipment.
Licensee procedure 30AC-9HP02,'Preventive Maintenance,"
Revision 2, dated Hay 1, 1990, which implemented requirements for performing preventive maintenance of safety-related equipment, requires in Section 3.4.2.2 that the'Maintenance Manager (or designee) approve in writing any delay of a preventive maintenance task beyond its grace period.
Section 4.1 of procedure 30AC-9HP02 defines "beyond its grace period" as, "When a
PM Work Order has NOT been completed and has exceeded the due date by 25X of the maintenance period."
Contrary to the above, on October 4, 1991, the NRC identified that preventive maintenance task 8024600 (calibration of Unit 1 Emergency Diesel Generator "A" phase differential relay) had not been performed since April 4, 1989, which was beyond its grace period, and without written concurrence from licensee management.
This is a Severity Level IV violation (Supplement I) applicable to Unit 1.
B.
10 CFR Part 50, Appendix B, Criterion V, requires that activities.
affecting quality shall be prescribed by documented instructions, procedures, or drawings of a type appropriate to the circumstances.
Procedure 41AL-1RK4A, "PZR PRESS HI-LO," provides instructions on the operator response to a pressurizer pressure hi/low alarm, an activity affecting quality.
Contrary to the above, as of June 2,
