NRC INSPECTION REPORT NO. 50-528/529/530/92-22
This refers to the inspection
conducted
by D. Coe,
F. Ringwald, J. Sloan,
and
L. Tran of this office, on Hay 31 through July 18,
1992.
The inspection
included
a review of'ctivities authorized for your Palo Verde facility.
At
the conclusion of the inspection,
the findings were discussed
with those
members of your staff identified in the enclosed
report.
Areas examined during the inspection are identified in the report.
Mithin
these
areas,
ihe inspection consisted of selective
examinations of procedures
and representative
records,
interviews with personnel,
and observation of
activities in progress.
Based
on the results of this inspection,
cer tain of your activities appeared
, t
b olation of NRC requirements.
These activities were associated
with
your preventive maintenance
program
and your procedures
to respondd to a stuck
open pressurizer
spray valve.
However,
based
on our review of your corrective
actions in each of these
cases,
we concluded that these
actions
were
appropriate
and that no further response
is warranted at this time.
As discussed
in the Notice of Violation and Proposed
Imposition of Civil
Penalty issued
on August 13,
1992,
and in paragraph
10 of this inspection
t
'nitial efforts to resolve the continuing problems with General
Electric reactor trip breakers
appeared
to be not as thorough or complete as
expected.
You may respond to this issue
as part of your response
to the
Notice of Violation and Proposed
Imposition of Civil Penalty dated August 13,
1992.
In accordance with 10 CFR Part 2.790 of the NRC's "Rules of Practice,"
a copy
of this letter and the enclosures will be placed in the
NRC Public Document
Room.
