REPORT

NO. 50-528/529/530/92-23

This refers to the special

inspection

conducted

by B. Olson

on June

15 through

June

19,

1992.

The inspection

included. a review of activities authorized for

your Palo Verde facility.

At the conclusion of the inspection,

the findings

were discussed

with those

members of your staff identified in the enclosed

report.

Areas examined during the inspection

are identified in the report.

Within

these

areas,

the inspection consisted

of selective

examinations of procedures

and representative

records,

interviews with personnel,

and observations

by the

inspector.

Based

on the results of this inspection,

one apparent violation was identified

and is being considered for escalated

enforcement

action in accordance

with

the "General

Statement of Policy and Procedure for NRC Enforcement Actions"

(Enforcement Policy),

10 CFR Part 2, Appendix C.

The apparent violation

involves the failure to maintain'a

high pressure

safety injection check valve

(a containment isolation valve) operable

in that its disc was installed

backwards for one operating cycle.

Accordingly,

no Notice of Violation is

presently being issued for these

inspection findings.

In addition,

please

be

advised that the number

and characterization

of apparent violations described

in the enclosed

inspection report

may change

as

a result of further

NRC

review.

The

NRC is also considering escalated

enforcement

action pertaining

to maintenance

and work practices

associated

with the Unit 3 loss of

annunciator

event

and the Unit 3 reactor trip breaker failures.

Our

inspection of these

events

was forwarded to you in NRC inspection reports

50-

528/529/530/92-15

and 50-528/529/530/92-19.

An enforcement

conference

to discuss

these

apparent violations has

been

scheduled for July 9,

1992.

The purposes

of this conference

are to discuss

the apparent violations, their causes

and safety significance; to provide you

the opportunity to point out any errors in our inspection reports; to provide

an opportunity to present your proposed corrective actions;

and to discuss

any

other information that will help us determine

the appropriate

enforcement

with the Enforcement Policy.

You will be advised

by

separate

correspondence

of the results of our deliberations

on this matter.

No response

regarding these

apparent violations is required at this time.
