04/11/92 Will refueling or resumption of operation thereafter require a Technical Specification change or other license amendment'?

Yes. Figures 3.2-2 and 3.2-2A of the Technical Specifications will require revision to reflect different DNBR margin limits.

Scheduled date for submitting proposed licensing action and supporting information.

12/2/91 Important Licensing considerations associated with refueling, 'e.g., new or different fuel design or supplier, unreviewed design or performance analysis methods, significant changes in fuel design, and new operating procedures.

The U1C4 reload analysis has been performed by ABB-CE using methodology from Topical Report CENPD-266-P-A, with revisions to biases and uncertainties for both the ROCS-NEM computer code and the net rod worth (N-1) calculation to include more recent measurements in the data base.

SUMMARY

OF OPERATING EXPERIENCE FOR THE MONTH DOCKET NO. 50-528 UNIT NAME PVNGS-1 DATE COMPLETED BY K.A. Chavet October 1991 10/01 0000 Unit began the month in Mode 1, 100% RX power.

10/07 0903 Commenced RX power decrease to 65% due to Feedwater Pump Turbine valve problems. 'A'heck 10/07 1050 RX power at 65%.

10/07 2315 Commenced RX power increase to 100%.

10/08 0615 RX power at 100%.

1.0/27 0722 Entered Mode .3 when the RX/turbine tripped as a result of a lightning-induced fault on a grid secondary feeder. A momentary power load unbalance caused fast closure of two main turbine control valves. The resultant change in steam flow caused Quick Open signals to the steam bypass control valves. The main turbine control valves then reopened, which combined with the steam bypass control valves being open, decreased primary temperature. The power increase associated with the decreased temperature caused a reactor trip on Variable Over-Power.

In addition, Safety Injection Actuation Signals (SIAS) and Containment Isolation Actuation System (CIAS) actuated due to low pressurizer pressure.

10/27 0729 Declared NUE due to SIAS.

10/27 0905 Terminated NUE.

10/31 0444 Entered Mode 2.

LO/31 1237 Entered Mode 1.

10/3L 1608 Main Generator Synchronized to the'rid.

10/31 2400 Ended the month in Mode 1, ascending to 100% RX power.

Unit was shut down on 10/17/91 for refueling.

Scheduled date for restart following refueling.

12/26/91 Will refueling or resumption of operation thereafter require a Technical Specification change or other license amendment?

No. Analyses indicate that no proposed Technical Specification change or other license amendment will be required as a result of the next refueling.

4. Scheduled date for submitting proposed licensing action and supporting information.

N/A Important Licensing considerations associated with refueling e.g., new or different fuel design or supplier, unreviewed design or performance analysis methods, significant changes in fuel design, and new operating procedures.

U2C4 is typical of PVNGS reload cycles.
