Dear Mr. Martin:
Subject:
Palo Verde Nuclear Generating Station (PVNGS)
Units 1, 2, and 3 Reexamination Material and List of Operators for Requalification Examination File: 91-005-419.06 Arizona Public Service Company (APS) provided, with Reference (A), a list of proposed operators (including crew composition) for the upcoming Requalification Program Evaluation scheduled to begin at PVNGS on September 9, 1991. Senior operator Steven L. Quan, who was included on the list enclosed with Reference (A), will not be examined during this evaluation visit. His position on Administrative Crew 1 will be filled by senior operator Erik M. Flodin. This change was discussed during a telephone conversation on August 1, 1991, between Mr. G. W. Johnston of your staff and representatives of the PVNGS Nuclear Training Department, and is reflected in Enclosure A.
Additionally, as requested by Mr. G. W. Johnston during the above cited telephone conversation, APS has provided to the NRC contractor, EG&G, appropriate reference material for use in developing the reexamination requested in Reference (B).
This reference material consists of the examination material previously provided to Pacific Northwest Laboratory-Battelle by copy of Reference (C), as well as 9108280256 '.01082k PDR ADOCK 05000528 lk V PDR I)
161-04121-JML/GEC August 21, 1991 Mr. John BE Martin U.S. Nuclear regulatory Commission Reexamination Material and List of Operators for Requalificati.on Examination Page Two additional examination material provided by APS to EG&G. Arrangements were made for the transfer of examination material from Pacific Northwest Laboratory Battelle to EG&G. This material was indexed in the Enclosure to Reference (C).
Additional material, indexed in Enclosure B, has been provided directly to Mr.
J. F. Hanek of EG&G by APS and includes supplementary examination material and replacement copies for those PVNGS procedures that were revised subsequent to the submittal of Reference (C). These replacement procedures should be substituted on a one-for-one basis for those previously provided by Reference (C). This material, taken together, constitutes the reference material for use in developing the reexamination.
