This letter refers to an enforcement

conference

that was held with yourself,

Mr. J. 'W. Bailey, and other members of your staff,

and the

Region

V staff, at the

NRC Region

V Office in Walnut Creek, California,

on

March 30,

1990 to discuss

the apparent violations that are

summarized

in

Region

V Inspection

Report

Nos.

50-528/90-04

and 50-528/90-13.

The

conference

is summarized

in Region

V Inspection

Report

50-528/90-18,

50-529/90-18

and 50-530/90-18 (Attached).

The focus of the enforcement

conference

was to discuss

the circumstances

surrounding

events

involving the control of locked high radiation areas

which

were found to be open,

unoccupied,

and unguarded

on three separate

occasions

in Units

1 and

3 between

the period of November

6,

1989 and February

22,

1990.

Based

on the results of the inspections

referenced

herein, certain of your

activities appeared

to be in violation of

NRC requirements,

as specified in

the Notice of Violation, enclosed

herewith

as Appendix A.

The apparent

violations, the root cause for the violations

and your corrective actions

were

discussed

with you during the enforcement

conference.

As we expressed

to you

at the enforcement

conference,

the repetitive nature of the violations,

the

lack of understanding

and/or inattention to detail for procedure

compliance,

and the ineffectiveness

of your past corrective actions indicate that the

causes

for the violations

may not have

been appropriately identified and

addressed.

We urge that attention

be directed

towards

improving personnel

performance

to ensure

the safety of licensed activities is maintained.

We

emphasize

that any similar violations in the future

may result in additional

enforcement

action.

The violations

have

been categorized

by severity level in accordance

with the

"General

Statement

of Policy and Procedure

for NRC Enforcement Actions",

10 CFR Part 2, Appendix

C (1989).

This Notice is sent to you pursuant to the

provisions of Section 2.201 of the

NRC's "Rules of Practice",

Part 2, Title

10,

Code of Federal

Regulations.

You are required to respond to this letter and should follow the instructions

specified in the enclosed

Notice when preparing your response.

In your

response,

you should document the specific actions

taken

and any additional

actions you plan to prevent recurrence.

After reviewing your response

to this

Notice, including your proposed corrective actions

and the results of future

inspections,

the

NRC will determine

whether further

NRC enforcement action is

necessary

to ensure

compliance with the regulatory requirements.

In accordance

with 10 CFR 2.790 (a),

a copy of this letter and the enclosures

will be placed in the

NRC Public Document

Room.

The response

directed

by this letter and the accompanying

NOV are not subject

to the clearance

procedures

of the Office of Management

and Budget

as required

by the Paperwork

Reduction Act of 1980,

PL 96-511.
