~Aendix A Notice of Violation Arizona Public Service Company Docket Nos. 50-528, 50-529, 50-530 Palo Verde Nuclear Generating Station License Nos.
NPF-41, NPF-51, NPF-74 Unit 2 During an inspection conducted on August 1-.11, 1988, three violations of NRC requirements were identified.
Two of the violations wi 11 be the subject of separate correspondence.
In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C, the remaining violation is listed below:
10 CFR 50.59(b)(l) states in part, that:
"The licensee shall maintain records of changes in the facility...made pursuant to this section, to the extent that these changes constitute changes in the facility as described in the safety analysis report....
These records must include a written evaluation...."
Sections 11.4 and 12.5 of the licensee's Final Safety Analysis Report (FSAR) identify systems and facilities that are within the site boundary that have been approved for handling and processing of radioactively contaminated material.
Contrary to the above, Trailer 3-2 was recently converted into a respiratory processing facility.
At the time of this inspection, the trailer was being used to temporarily store and process radioactively contaminated respirators.
As of August 11,
- 1988, the licensee had not performed a written safety evaluation addressing the processing of radioactive equipment in Trailer 3-2.
This is a Severity Level IV Violation (Supplement 1).
Pursuant to the provisions of 10 CFR 2.201, Arizona Public Service Company is hereby required to submit a written statement or explanation.to the U. S.
Nuclear Regulatory Commission, ATTN:
Document Control Desk, Washington, DC 20555 with a copy to the Regional Administrator, Region V, within 30 days of the date of the letter transmitting this Notice.
This reply should be clearly marked as a "Reply to a Notice of Violation" and should include:
(1) the reasons for the violations, if admitted, (2) the corrective steps that have been taken and the results
- achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be achieved.
1f an adequate reply is not received within the time specified in this Notice, an order may be issued to show cause why the license should not be modified, suspended or revoked or why such other action as may be proper should not be taken.
Consideration may be given to extending the response time for good cause shown.
