NRC Inspection of Palo Verde Unit

1

This refers to the special

inspection of July 5 through July 8,

1988,

conducted

by Mr. T. Polich, of activities authorized

by NRC License

No.

NPF-41

and to the discussion of our findings held with you and members of

your staff on July 8, 1988, at the conclusion of the inspection.

The

purpose of the inspection

was to review the circumstances

which resulted

in both trains of the Essential

Chilled Water System being rendered

inoperable for approximately nine days, with Unit

1 in power operation in

violation of the Technical Specifications.

Areas

examined during this inspection

are described

in the enclosed

inspection report.

Within these

areas,

the inspection consisted

of

selective

examinations

of procedures

and representative

records,

interviews with personnel,

and observations

by the inspector.

Based

on the results of this inspection, it appears

that one of your

activities was not conducted

in full compliance with NRC requirements.

This violation will be the subject of separate

correspondence.

We concluded that the cause of this event

was several

consecutive

instances

of poor communication

between different members of your

operations staff, coupled with a reluctance

on the part of

the'perator

who ultimately rendered

the system inoperable to follow up

on his thoughts that 'something

was wrong.

There

have

been several

other recent events

which have reflected poorly on operator

performance,

including the early criticality at Unit

1 on May 14,

1988; the erroneous

operation of the Unit 1" main generator

motor operated

disconnect

on

April 19, 1988, which. resulted in a reactor trip; the entry into

Mode 4 at Unit 1 on February 29,

1988 without an operable

High Pressure

Safety Injection pump,

as required

by the Technical Specifications;

the

operation of Unit 2 in February,

1988, with less

than the required

number of Auxiliary Feedwater

Pumps;

and the inadvertent safety

injection at Unit 2 on February 21, 1988.

Several of the above events

contributed to a civil penalty enforcement action which was

imposed in

April, 1988.

In transmitting that enforcement

action to you, we observed

that operator

performance

and your management

expectations

were divergent.

The poor practices

which appear to have caused this event indicate that

operator

performance

remains at

a level below that desired.
