and other members

of your staff and myself, Mr.

F. J. Miraglia and other members of the

NRC

staff at the

ANPP Corporate

Engineering Office in Phoenix,

Arizona,

on'ugust

12,

1987.

The subjects

discussed

are

summarized

in Meeting Report

Nos. 50-528/87-30,

50-529/87-30,

and 50-530/87-31,

enclosed

herewith for your

information.

The discussion primarily centered

on your evaluation of recent events,

NRC

concerns

generated

from the assessment

of the operating performance of Palo

Verde Units 1 and

2 in consideration

of the pending licensing decision for

Unit 3 and to identify needed

licensee

actions

for improvement prior to the

licensing decision.

Aspects of previous

issues

discussed

at our May ll,

1987, meeting at the

NRC Region

V Office were also discussed

and updated to

current status.

Two areas

of improvement were noted.

The root cause

analysis of events

has

improved in depth

and attention to detail

and the performance

indicators

have

generally

improved.

However, personnel

errors

have started to creep

up.

Management attention is needed to stabilize this increase

in personnel

errors

and reestablish

a downward trend.

It is particularly important to

ensure that the reorganization

and attendant

confusion is not allowed to

diffuse your efforts in this area.

In light of the problems with fuel integrity,

ANPP needs

a well thought out

program for handling hot particles,

and you need to carefully assess

the

plant capacity to process

water with high levels of activity.

The high continuing level of open maintenance

work orders without a well

focused

program for improvement is considered

a significant problem.

Also,

the simulator training provided for licensed operators

appears

to be at the

minimum level.

This, in addition to the recent "marginal" rating for your

licensed operator requalification program,

indicates

a lack of attention

by

the appropriate

levels of management.

You should

be prepared

to discuss your

planned

improvements

in these

areas

during the

NRC Commission meeting for the

Unit 3 full power license.

As with our past meetings,

we found this meeting to be beneficial with good

communication

between

ANPP and the

NRC.

We look forward to discussing

the

effectiveness

of your corrective actions during the scheduled

NRC Commission

meeting

and at a future meeting of this type.
