OF MEETING WITH ANPP MANAGEMENT

This refers to a meeting .held with you and members of your staff at the

Arizona Public Service

Company corporate offices in Phoenix, Arizona on

December

1, 1989.

The subjects

discussed

are

summarized

in Meeting Report

50-529/89-55,

and 50-530/89-55,

enclosed

herewith.

A copy

of the slides

used during your presentations

is also enclosed.

During this meeting

we discussed

with you the Systematic

Assessment

of

Licensee

Performance

(SALP) results for Palo Verde which we had transmitted to

you in Report

Nos. 50-528/89-48,

50-529/89-48

and 50-530/89-48.

We reiterated

our assessment

of your performance for the

SALP period

as

one that appeared

to

decline during the first half of the period

and that appeared

to improve

during the

second half of the period.

Your efforts in strengthening

your

management

team

and in initiating your various

improvement

programs

appeared

to have started your recovery toward better performance.

However,

we also

were cautiously optimistic in that the results of your improvement

programs

are still forthcoming

and will require vigorous management

attention to

achieve

the desired

improvement.

You acknowledged

our

SALP report

and agreed

in general

with its content.

During this meeting,

we also took the opportunity to discuss with you the

status of your Unit 3 pre-restart

action items.

We were encouraged

to hear

the significant strides

you have

made in accomplishing

and verifying completion

of many of these

items.

However,

we also recognized that significant effort

is still needed

to complete this work.

We reiterated

our expectations

that

you provide the necessary

support to your operators

by correcting the problems

they identify, such

as discrepancies

with your control

room annunciators,

instrumentation

and other items that have

been

on your control

room discrepancy

logs for a long period of time.

During the meeting,

we also discussed

your

manageme'nt

review committee activities, the role of OA in your organization,

and your engineering

improvement initiatives.

Finally, we reviewed with you recent events at Palo Verde such

as the Unit

1

stuck fuel assembly,

the Unit 2 reactor trip of October 31,

1989, the Unit 2

excessive

heat-up

on November 21,

1989 and the problems you have

had with your

Auxiliary Feedwater

motor operated

valves in Units

2 and 3.

We discussed

the

apparent

continuing problem you have

had with regard to the lack of management

involvement in important operational

decisions

and

an apparent

consequent

recurrence

of the practice of proceeding with an operation without full

knowledge of the available

information that would allow you to resolve

problems

encountered,

as

was

the case

during the Unit

1 stuck fuel assembly evolutions.

In accordance

with 10 CFR 2.790(a),

a copy of this letter and the enclosures
