and Construction Permit Number CPPR-143,

and to the discussion of our findings

held with members of your staff at the conclusion of the inspection.

Areas

examined during this inspection are described in the enclosed

inspection

report.

Within these

areas,

the inspection consisted of selective

examinations of procedures

and representative

records,

interviews with

personnel,

and observations

by the inspectors.

Recent

cases of personnel error'iscussed

in paragraph

15 of the enclosed

report, indicate the continuing need for aggressive

management

action to

minimize personnel

errors in the future.

Previous

NRC inspection reports

have

documented

our concern regarding problems during the operational

phase

which

were attributed to instances

of personnel error, including the failure to

follow plant procedures.

Your letter to the Region V office, dated

July 7,

1986 communicated

the corrective actions undertaken

and planned to

improve plant performance in this area.

Although some

improvement

has

been

evident,,

the recent

examples

indicate that further improvement is necessary.

In our meeting 'on October 28,

1986, it was tentatively agreed to review this

.area,

among others, in another

management

meeting in Walnut Creek in about 4-6

weeks.

At that time I would appreciate

your analysis of the effectiveness

of

your efforts to reduce personnel errors.

No violations or deviations of NRC requirements

were identified within the

scope of this inspection.

In accordance

with l0 CFR 2.790(a),,

a copy of this letter and the enclosure

will be placed i.n the

NRC Public Document

Room.
