Review of Palo Verde Procedures as Related to the July 12th Trip at Palo Verde Unit 1

In response to questions raised by R.

Zimmerman, Senior Resident Inspector at Palo Verde Nuclear Generating Station, NRR staff members have conducted a

review of actions'aken by the Palo Verde control room staff in dealing with an event that took place at Palo Verde Unit 1 on July 12, 1986.

This review involved an analysis of the Palo Verde Emergency Operating Procedures with the Combustion Engineering Procedures guidelines (CEN-152),

an examination of the post-trip review of the event, and the evaluation of operator actions as the reviewers understood them following discussions with plant staff and the senior resident inspector.

The results of this evaluation have been provided to Region V and are enclosed for your information.

One of these findings is that your procedures program did not appear to meet commitments established during the licensing process.

These "deviations" are related to the EOP's and crew response to the July 12, 1986 event as identified in Section 1.,

page 2 8 3 of the staff evaluation.

Furthermore, actions and instructions in the Diagnostic Flow Chart associated with tripping the reactor coolant pumps appear to be inconsistent with the current approved version of CEN-152 and with the plant's written procedures.

In addition, the use of parameter trending as indicated in CEN-152 to assist in event of diagnosis does not appear to be effectively incorporated into the Diagnostic Flow Chart, as indicated in Section 2 of the evaluation.

The reviewers have offered a number of recommendations for addressing the issues identified.

These recommendations suggest the need for a more reliable power source for instrumentation displaying the parameters necessary for event diagnosis (Section 3), and revisions to the Diagnostic Flow Chart to ensure that the proper sequence of steps are followed and that the Functional Recovery Pro-cedures are used when necessary.
