REVIEW 0F PALO VERDE PROCEDURES AS RELATED TO JULY 12 TRIP AT PALO VERDE As requested, we have conducted a review of actions taken by the Palo Verde control room staff in dealing with an event that took place at Palo Verde Unit 1 on July 12, 1986. This review involved an analysis of the Palo Verde Emergency Operating Procedures, a comparison of those procedures with the Combustion Engineering Emergency Procedure Guidelines, an examination of the post trip review of the event, and an evaluation of operator actions as we understood them following discussions with plant staff and the senior resident inspector.

Based on our review, it appears that there are a number of deficiencies in the l,

Palo Verde Emergency Operating Procedures, the plant operating philosophy, and the generic guidelines. We have included in our evaluation recommendations for corrective action in dealing with these apparent deficiencies. We propose to transmit our report to both Palo Verde and Combustion Engineering, so that they may consider actions which would be appropriate in light of our findings.
