APPENDIX A NOTICE OF VIOLATION Arizona Public Service Company P.
O. Box 21666 Docket No. 50-528 Construction permit 141 As a result of the inspection conducted on August 13 - September 8, 1984,.and in accordance with NRC Enforcement Policy, 10 CFR Part 2, Appendix C, 47 FR 9987, the following violation was identified.
10 CFR 50, Appendix B, Criterion V, Instructions, Procedures and Drawings states in part "Activities affecting quality shall be prescribed by documented instructions, procedures,'r drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures or drawings."
APS Operations equality Assurance Criteria Manual, Criterion 5, Instructions, Procedures and Drawings, Revision 1, dated August 23, 1984 states that quality-related activities shall be accomplished in accordance with applicable instructions, procedures or drawings.
Contrary to the above, on August 27, 1984 primary coolant chemical addition was attempted without a procedure to assure proper valve alignment.
This resulted in an unplanned overfill of the reactor vessel.
This is a Severity Level IV Violation (Supplement II).
