Confirmatory Information Required to Complete Staff PVORT Audit Review Following the PVORT Audit that was held on August 3-6, 1982 at the Palo Verde qite, the staff has identified five confirmatory items for which additional information is required to complete its review.

The items, as well as the required information, are discussed in Enclosure l.

He request that you provide the information identified in Enclosure 1.

In addition, since the qualification program for the safety-related pumps and valves was not 100Ã complete at the time of the staff audit, we request that you provide a schedule for completion and confirm prior to fuel load for Palo Verde Unit 1 that the program is complete.

Confirmator Items To Com lete Staff PVORT Audit Review of Palo Verde The following five items (3 plant generic and 2 equipment specific) identify the confirmatory information that is required by the staff to complete its review of the PVORT audit for Palo Verde.

Plant Generic Items APS does not have ade uate administrative rocedures to ensure that certain valves will not be "manuall disarmed."

APS addressed the problem of the.Rotork and Fisher valves being "manually disarmed" by presenting its policies (i.e, administrative procedures) which

,would aid in insuring.that valves would not be -".manually disarmed" if they were required to perform a safety function.

The two most important safeguards are; (a) each valve will be stroked remotely after maintenance to verify proper operation, and (b) operations personnel will perform a routine valve line-up verification check at least once every 31 days.

These two safeguards are not in themselves sufficient to insure proper operation of the applicable valves.

The valve line-up check list must include special instruction for all valves that have the potential to be "manually disarmed."

For example, instructions for a Rotork valve might state, "Verify that V-XXX is in the open (or closed) position and that the manual handwheel lever is not secured in the manual position."

Instructions for a Fisher valve might state, "Verify that V-XXX is in the open (or closed) posi-tion and that,the manual handwheel is in. the closed (or open) position."

In addition, every valve that has the potential to be "manually disarmed" should be labeled to that effect, on the valve line-up check list, by means of a Warning Notice which explains the potential problem to the operations personnel performing the valve line-up check.

APS should include these provisions in the. valve line-up verification check list and a copy of the, revised check list should be provided to the NRC for confirmation.

At resent APS must rel on its contractors to locate certain uglification documents.

A record of qualification must be maintained in an "auditable form" but not necessarily in a central file for the entire period during which the covered item is installed in a nuclear power plant.

Recordkeeping require-ments of 10 CFR Part 50 Appendix B must be met.

Certain records can be kept at the vendor's shop.

A review of the APS files indicated that all the.,documents relating to the Pump and Valve Operability Assurance Program are hot stored with APS.

APS had difficutly in using a vendor cross-reference system to retrieve certain documents in the file.

APS must provide confirmation that all qualification documents are readily locatable, either with APS or at the vendor.

An overview of APS Station Information Management System (SINS) was presented.

SINS is a computer based system used to schedule preventive maintenance and surveillance tests.

Work in that area was on-going during the audit.

Therefore, APS must confirm that all pumps and valves that provide a

safety function are included in SIt1S.

S IA-UV-655 The concern regarding this valve assembly is that documentation was not available to verify that it would open against a maximum differential pressure.

APS addressed this concern by presenting the Palo Verde Nuclear Generating Station Manual, Procedure No. 91PE-1SI03, Revision 0, entitled "Shutdown Cooling Test",

and stating that this test would be conducted as part of the station's preoperational testing program.

Confirmation that this test was successfully performed must be provided to the NRC.

SGE'-'UV-180 The pre-operational testing procedure addressed stroking of the valve, but not under flow conditions.

APS assured the staff that the stroking (of-the-valve under full'flow conditions~viould".be accomp1~ished"during:-hot functional testing.

Confirmation that this test was successfully performed under full flow conditions must be provided to the NRC.
