- 141, 142 NOTICE OF VIOLATION As a result of the inspection conducted on'ugust 17-21,
- 1981, and in accordance with the Interim Enforcement Policy', 45 FR 66754 (October 7, 1980), the following violations were identified:
Appendix B of 10 CFR 50, Criterion V, as addressed in paragraph 5.1 of the guality Assurance Pr ogram in Appendix A to the PSAR, states in part:
"Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions procedures, or drawings...."

The Bechtel guality Assurance Manual ASME Section III, Division 1, 1977 Edition, Section 5000, Paragraph 5148, states in part:
"Requirements for receipt, inspection, storage...are in ltFMC-1.
As a minimum WFMC-1 requires that...filler material be kept...
from... unwanted moistur e,"
Contrary to the above, it was noted on August 20, 1981 that in Unit 2 Rod Room No. 4, unwanted moisture (lunch) was found in one rod oven containing low hydrogen electrodes.
This is a Severity Level VI Violation (Supplement II) and is applicable to Docket No. 50-529, 2.
Bechtel MPP/gC1-13.0, "Housekeeping",
requires that access control be maintained in zone IV designated areas.
Contrary to the above, uncontrolled access was observed on August 17, 1981 in the Unit 1 zone IV designated fuel pool area from the fuel transfer tube area into the fuel pool; and on August 20, 1981 in the Unit 1 zone IV designated fuel pool area through an open door above the fuel pool.
This is a Severity Level YI Violation (Supplement II) and is applicable to Docket Nos. 50-528 and 50-529.

Bechtel NPP/gCI-58.0, "Erection of Structural and Miscellaneous Steel", section 4.4.4 states:
"Where long slotted holes are used on an outer ply, a plate washer or continuous bar of at least 5/16 inch thickness with standard holes shall be provided.
These washers or bars shall have a size sufficient to completely cover the slot after installation."
