NOTICE OF VIOLATION Washington Public Power Supply System Washington Nuclear Project-2 Docket No.:

NPF-21 During an NRC inspection conducted on February 10-14, and March 19, 1997, two violations of NRC requirements were identified.
In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," NUREG-1600, the violations are listed below:
A.
10 CFR Part 50, Appendix B, Criterion III (Design Control) states, "Measures shall be established to ass'ure that applicable regulatory requirements and the design basis...
are correctly translated into specifications, drawings, procedures, and instructions."
Contrary to the above, as of January 30, 1996, the licensee failed to assure that
.applicable regulatory requirements for assuring that, safety-related pumps would operate as required were correctly translated into specifications.and procedures.
Specifically, Plant Modification Record (PMR) 85-0528-0, "Protective Relay coordination Study and Installation" established an instantaneous overcurrent trip setpoint for Service Water Pump 1A and.the low pressure core spray pump which was nonconservative and were too close to actual pump motor starting currents.
This resulted in the trip of a Service Water Pump 1A on a routine start.
This is a Severity Level IV violation (Supplement I).

I WNP-2 Technical Specification 6.8.1a states, in'art, that written procedures. be established, implemented, and maintained covering the applicable procedures recommended in Appendix A of Regulatory Guide 1.33, Revision 2, dated February 1978..
Regulatory Guide 1.33, Revision 2, specifies piocedures for the repair or replacement of equipment.
Plant Procedure Manual 1.15.12, Section 8.4, Revision 0, "Substitution Evaluation,"

Contrary to the above as of April 26 and 29, 1996, a substitution evaluation was not requested to be performed prior to the installation of substitute high pressure core spray drywell Pressure Switches MS-PS-47A, 47B, 47C, and 47D.

. Pursuant to the provisions of 10 CFR 2.201, Washington Public Power Supply System is hereby required to submit a written statement or explanation regarding Violation A to the U.S. Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington, D.C.
20555 with a copy to the Regional Administrator, Region IV, 611 Ryan Plaza Drive, Suite 400, Arlington, Texas 76011, and a copy to'the NRC Resident Inspector at the facility that is the subject of this Notice, within 30 days of the date of the letter transmitting this Notice of Violation (Notice). This reply should be clearly marked as a "Reply to a Notice of Violation" and should include for each violation:
(1) the reason for the violation, or, if contested, the basis for disputing the violation, (2) the corr'ective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the. date when full compliance will be achieved.
Your response may reference or include previous docketed correspondence, if the correspondence adequately addresses the required response.
If an adequate reply is not received within the time sp'ecified in this Notice, an order or a Demand for Information may be issued as to why the license should not be modified, suspended, or revoked, or why such other action's may be proper should not be taken.
Where good cause is shown, consideration will be given to extending the response time.
The NRC has concluded that. information regarding the reason for Violation B, the corrective actions taken and planned to correct the violation and prevent recurrence, and the date when full compliance was achieved is already adequa'tely addressed on the docket in NRC Inspection Report 50-397/97-04, Enclosure 2, However, Washington Public Power Supply System (Licensee) is required to submit a written statement or explanation pursuant to the provisions of 10 CFR 2.201 if the description therein does not accurately reflect the
,Licensee's corrective actions or position.
In that case, or if the Licensee chooses to
- respond, clearly mark the response as a "Reply to a Notice of"Violation" and send it to the U.S. Nuclear Regulatory Commission, ATTN: Document Control,Desk, Washington, D.C.
