NRC INSPECTION REPORT 50-397/95-19
This refers to the inspection
conducted
by Hessrs.
S.
L. HcCrory,
R.
E. Lantz.
T. 0. HcKernon,
and
D. L. Proulx, of this office on June 5-20,
1995.
The
inspection included
a review of activities authorized for your Washington
Nuclear Project-2 facility.
At the conclusion of the inspection,
the findings
were discussed
with you and those
members of your staff identified in the
enclosed
report.
Areas
examined during the inspection are identified in the report.
Within
these
areas,
the inspection consisted of selective examinations of procedures
and representative
records,
interviews with personnel'nd
observation of
activities in progress.
The purpose of the inspection
was to confirm that
control
room operating
crews were performing in accordance
with your
management
expectations
and procedures
and in accordance
with NRC
requirements.
While we concluded that operations
performance
was adequate
to protect public
health
and safety.
we remain concerned with the generally
low level of
operations
performance
and the lack of an established
improving trend.
A
number of long standing conditions which contribute to poor performance
were
still in evidence.
Based
on the results of this inspections
certain licensed activities appeared
to be in violation ot
NRC requirements,
as specified in the enclosed
Notice of
Violation (Notice).
The violation is of concern
because it was identified by
the
NRC.
Horeover, it typified previously identified weaknesses
regarding
procedural
adequacy,
information loss across shift turnover, corrective
actions,
and rigor concerning firm and discretionary
requirements.
You are required to respond to this letter and should follow the instructions
specified in the enclosed
Notice when preparing your response.
In your
response.
you should document the specific actions taken
and any additional
actions you plan to prevent recurrence.
Your response
may reference or
include previous docketed
correspondence. if the correspondence
adequately
