NOTICE OF VIOLATION Washington Public Power Supply System Docket:
50-397 Washington Nuclear Project 2
L'icense:
NPF-21 During an NRC inspection conducted on June 5-20.
- 1995, a violation of NRC requirements was identified.
In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix CD the violation is listed below:
A.
Technical Specification 6.8. 1 states'n part.
"Written procedures shall be established,"
implemented, and maintained covering the activities referenced below:
1.
The applicable procedures recommended in Appendix A of Regulatory Guide 1.33, Revision 2, February 1978.
2.
The applicable procedures requi red to implement the requi rements of NUREG-0737."
Regulatory Guide 1.33, Appendix A, Revision 2, February 1978 lists a
procedure for "Shift and Relief Turnover."
NUREG-0737 'ection 1.C.2, states, "The licensees shall review and revise as necessary the plant procedure for shift and relief turnover to assure the following.
a checklist shall be provided for the oncoming and offgoing control room operators and the oncoming shift supervisor to sign."
Technical Specification 6.8.2 states, "Each procedure of Specification-6.8. 1.
and changes
- thereto, shall be reviewed by the POC LPlant Operations Committeej and shall be approved by the Plant Manager prior to implementation and reviewed periodically as set forth in administrative procedures."
Contrary to the above, as of June 9, 1995, the licensee did not maintain a Plant Operations Committee approved procedure for shift and relief turnover that included a shift and relief turnover checklist.
This is a Sever ity Level IV violation (Supplement I) (397/9519-01).
Pursuant to the provisions of 10 CFR 2.201.
Washington Public Power Supply System is hereby required to submit a written statement or explanation to the U.S. Nuclear Regulatory Commission.

This reply should be clearly marked as a "Reply to a Notice of Violation" and should include for each violation:

that have been taken and the results achieved.
(3) the corrective steps that will be taken to avoid further violations.
and (4) the date when full compliance wi 11 be achieved.
Your response may reference or include previous docketed correspondence, if the correspondence adequately addresses the required response.
If an adequate reply is not received within the time specified in this Notice.
an order or a Demand for Information may be issued as to why the license should not be modified, suspended, or revoked.
or why such other action as may be proper should not be taken.
Where good cause is shown. consideration wijl be given to extending the response time.
