NRC inspectors
on May 16

1988, at the
R.
E.
Ginna Facility, Ontario,
The team's findings were discussed
with
Mr. S. Spector of your staff at the conclusion of the inspection.
The
inspection results
are documented
in the enclosed
report.
In general
our inspectors
noted
an adequate
level of management
attention in
most areas
of IST program implementation.
Pump vibration monitoring methods,
establishment
of valve stroke time limits and good coordination
between test
and operations
personnel
during tests
were
some of the strengths
noted by the
inspection
team.
However,
we are concerned
about several
matters described
in
Section 3. 1 of the report involving the omission of certain safety related
valves from your IST program
and the inadequate. testing of certain
to meet the requirements
of ASME Code,
Section XI.
The inspection
team's
findings pertinent to the omission
from your IST program of the auxiliary
pump turbine main steam line check valves
and the main feedwater line
have credible safety significance which was not properly considered
by your staff in developing the program.
While we noted that these
apparent violations predominantly concerned
check
valves,
we are concerned that omissions
and deficient testing
may exist for
other valve types
and
pumps associated
with the IST program.
Since
we view
these matters to be serious,
we are considering further enforcement action.
Consequently,
we have
scheduled
an Enforcement
Conference
on June
24,
1988.
At this meeting,
you should
be prepared
to discuss
as
a minimum the following
items:

The root cause for the main
steam line check valve (3504B) failure and the
omissions of the auxiliary feedwater

from the IST program observed
by the inspection
team;

The safety significance of each of these 'findings relative to the plant
accident
analyses;
and,
3.
The corrective actions (short and long term) taken
and planned to prevent
recurrence.
In part'icular, this discussion
should entail your plans for a comprehensive
program review to provide the assurance
that other safety related valves
and
pumps are not omitted from the IST program or deficiently tested.
In this
regard,
we recognize
your commitments that were provided in your correspondence
of May 27,
1988 to Region I to address
immediate regional
management
concerns.
Any corrective actions that are
deemed
necessary
consistent
with these
commitments
should
be implemented without awaiting the enforcement
conference.
