RII:JAB 50-335/79-14 Findinn As required by Technical Specification 6.8.l.a,written procedures are required to be established, implemented and maintained covering the applicable proce-dures recommended in Appendix "A" of Regulatory Guide 1.33,

Contrary to the above:

Plant Operating Procedure 0410020 "HPSI/LPSI-Normal Operation", Section 8.1 was not followed in that safeguards pumps minimum flow isolation valve NV-3660, required to be locked open, was found open but unlocked on June 7, 1979.
2.
Plant Operating Procedure 0420020 "Containment Spray-Initial Valve Alignment",
Section 8.1.9 divas not followed in that B header eductor inlet valve 07261, required to be locked open, was found open but unlocked on June 7, 19/9.
~Res ense The valves required to be locked are now locked.,The individuals responsible for valve locking have been apprised of the importance of the finding.
The pre-start check list procedure (Administrative Procedure 0030120) will be revised by August 1, 1979.
The revised procedure will require verification of the locked valve list for a particular mode of operation prior to entry into that mode.
This requirement will be mandatory after any outage involving si gnificant val ve mani pul ation.
