NOTICE OF VIOLATION Commonwealth Edison Company Docket No~ 50-237 Docket No. 50-249 As a result of the inspection conducted -On July 14 through September 9, 1988, and in accordance with the General Policy and Procedure for NRC Enforcement
_Actions, "10 CFR Part 2, Appendix C" (1987), the following violation was identified:
- Dresden Technical Specification 3.7.C.1 requires, in part, that secondary containment integrity be maintained during all modes of plant operation except when all of the following conditions are met; (a) the reactor is sub-critical with proper shutdown margin, (b) reactor water temperature is* below 212 degrees F and reactor coolant system vented, (c) no ~ctivity.
is being performed which can reduce the shutdown margin below the Technical Specification 3.3.A. limits, and (d) the fuel cask or.
irradiated fuel is not being moved in the Reactor Building.
Contrary to the above, the Dresden Unit 2 reactor operated at power in*
the run mode**from May 5, 1988, to May 15, 1988, with a degraded secondary containment, in that, the Reactor Building to Turbine Building pipe.
penetration boot ~eals associated with Unit 3's four Main Steam Line penetrations, which are required to maintain s~condary containment for
- both* _Unit 2 and 3, were removed without proper authorization or knowledge of the Shift Supervisor.
In addition~ a further *breaching of secondary" containment integrity occurred during this same period.
On May 13, 1988,
- both_ the inner and outer Unit 3 Reactor Building to Turbine :Building interl~ck doors were opened simultaneously for approximately 10 minutes.
This is* a Severity Level IV violation (Supplement I) *.
The inspection. showed that action* had been taken to correct *the identified viol at ion and to prevent recurrence. Consequently, no. reply to this viol at ion is required and we have no further questions. regarding this matter.
