SUBJECT:
RADIATION PROTECTION AND RADWASTE INSPECTION AT DRESDEN
This refers to the routine safety inspection conducted by Mr. M. A. Kunowski
of this office from July 19 - 29, 1993.
The inspection included a review of
authorized activities for your Dresden Nuclear Station, Units 2 and 3.
At the
conclusion of the inspection, the findings were discussed with the members of
your staff identified in the enclosed report. This also refers to the
Management Meeting held at Dresden on July 12, 1993, to discuss the
effectiveness of radiological controls during the recent Unit 2 refueling
outage.
Areas examined during the inspection are identified in the report. Within
these areas, the inspection consisted of a selective examination of procedures
and representative records, observations, independent dose rate measurements,
and interviews with personnel.
The inspection identified a number of issues which are of concern to us:
An increase in the scope of work late in the planning stages for the
Unit 2 outage resulted in limited consideration of the exposure impact
of the work.
Additionally, planning and implementation problems
occurred on several high dose jobs, such as the reactor water cleanup
pipe replacement.
Consequently, the actual outage exposure more*than
doubled the estimate of 600 person-rem (6 person-Sievert).
Non-outage daily doses are relatively high due to high dose rates in
several general area walkways.
Poor oversight of cleanup personnel during the outage and t_he lack of
timely response to identified leaks resulted in numerous low-leveJ
contamination events. *
Corrective actions for the mispositioning of the drywell temporary
ventilation system were ineffective.

These examples show that the management of the radiological control program
has been ineffective in several areas.
Please provide, as part of your
resprinse to this letter, your asse~sment of this program and any planned
actions you may have for program improvement.
Based on the results of this inspection, certain of your activities appeared
to be in violation of NRC requirements, as specified in the enclosed Notice of
Violation (Notice). Corrective actions for two recent similar problems were
not effective in preventing this violation.
In your response, please discuss
why your proposed corrective actions will be more effective than your previous
actions.
You are required to respond to this letter and should follow the instructions
specified in the enclosed Notice when preparing your response.
In your
response, you should document the specific actions taken and any additional
actions you plan to prevent recurrence.
In addition to the required response
to the violation, you are also requested as noted above, to respond to the
concerns on management of your radiological control program and to the
effectiveness of corrective action for the violation. After we have reviewed
your response, I believe it would be beneficial to conduct another Management
Meeting with our respective staffs to further discuss this matter.
We will
contact your staff at a later date to arrange the meeting.
In accordance with 10 CFR 2.790 of the Commission's regulations, a copy df
this letter, the enclosures, and your response to this letter will be placed
in the NRC Public Document Room.
