NRC INSPECTION REPORT 50-387/98-12, 50-388/98-12;
Dear Mr. Byram:
i.
On January 4, 1999, the NRC completed an inspection at your Susquehanna
Steam Electric
Station (SSES) Unit 15 2 reactor facilities. The inspections covered routine activities by
the resident inspectors.
The enclosed report presents the results of these inspections.
The
inspectors discussed the findings of these inspections with Mr. R. Saunders,
Vice President
Nuclear Operations, Mr. G. Kuczynski, General Manager SSES, and others of your staff, at
an exit meeting at the completion of the inspection period.
During the 6-week period of inspection, your conduct of activities was characterized
by
safe operation and conservative decision making.
The station response to the Unit 1
offgas system isolation, Unit 1 main steam isolation valve fast stroke time, and Unit 2
standby liquid control tank foreign material removal was excellent.
However, over the past
six months we have noticed four occurrences of incorrect implementation of applicable
Technical Specification Limiting Conditions of Operation (LCOs) ~ The number of
occurrences
of incorrect implementation of LCOs, which have been minor in nature and non
consequential to date, appear to be an indicator of a potential adverse trend.
Based on the results of this inspection, the NRC has determined that a violation of NRC
requirements occurred.
On two separate
occasions incorrect gasket material was
substituted for a neoprene gasket on the "A" emergency diesel generator fuel oil storage
tank.
The violation.is cited in the enclosed Notice of Violation (Notice) and the
circumstances
surrounding it are described in detail in the subject inspection report.
The
violation is of concern because it indicates weaknesses
in your design control program

which were evident in your work planning process,
and it was identified by the NRC.
You are required to respond to this letter and should follow the instructions specified in the
enclosed Notice when preparing your response.
The NRC will use your response,
in part,
to determine whether further enforcement action is necessary to ensure compliance with
regulatory requirements.
