NRC INSPECTION REPORT 50-387/97-03, 50-388/97-03 AND

NOTICE OF VIOLATION

Dear Mr. Byram:

On May 19, 1997, the NRC completed

an inspection at the Susquehanna

Steam Electric

Station (SSES) facility. The enclosed report presents the results of that inspection.

During the 6-week period covered by this inspection, the conduct of activities at the SSES

facility was characterized

by safe operation and conservative

decision making.

In one

instance,

a Shift Supervisor made conservative

and technically correct decisions when

faced with conflicting procedural emergency

plan guidance.

We noted that operations

department management

involvement in this later issue was aggressive,

conservative,

and

considered

a strength.

Efforts by your staff to investigate and resolve issues with 4kV

breaker trip mechanisms,

a main steam isolation valve component problem, and a spent

fuel pool foreign material issue were also considered

strengths.

However, based on the results of this inspection the NRC has also determined that three

violations of NRC requirements

occurred.

The violations concern:

(1) two examples of

inadequate

procedural guidance;

(2) the failure of operators to properly perform an as-

found test of the core spray system; and (3) four examples of failure to perform a safety

evaluation prior to a design change.

These violations are cited in the enclosed

Notice of

Violation (Notice), and the circumstances

surrounding the violations are described

in detail

in the enclosed report.

Please note that you are required to respond to this letter and

should follow the instructions specified in the enclosed

Notice when preparing your re-

sponse.

The NRC will use your response,

in part, to determine whether further

enforcement action is necessary to ensure compliance with regulatory requirements.

Also of concern during this period were two separate

maintenance

related activities that

could have resulted in serious personnel

injury. One activity was identified by the Resident

Inspector and the other activity revealed itself when a load was dropped from a hoist.

Even though there were no violations of NRC requirements,

each of these activities could

have had significant consequences

and are further examples of the concern expressed

in

the previous NRC resident inspection report.

It was noted that Susquehanna

plant

management

responded

well to each activity from a corrective action and investigation

standpoint.
