This letter refers to the announced
inspection
conducted
by Mr. H. J.
Kaplan
and Mr.
M. Oliveri of this office on August 7-8,
1989 at the Susquehanna
Steam
Electric Station Unit
1 in Berwick, Pennsylvania.
Messrs.
Kaplan
and Oliveri
discussed
the findings of the inspection with Mr. Blakeslee of your staff at
the conclusion of the inspection.

The areas
examined during this inspection
are described
in the
NRC Region I
Inspection
Report which is enclosed with this letter.
The objective of the
inspection
was to determine
the events
and the reasons
that led to starting
up
the plant with potentially unacceptable
ultrasonic indications in the reactor
vessel.
The inspectors
determined*that
the problem occurred
because
of
inadequate
procedural
controls, within both your organization
and your inspec-
tion contractor's
organization that resulted in poor communication
between
these parties.
It should
be noted that in assessing
this issue
the
NRC
recognized that subsequent
to restart of the plant your. organization
recognized
the potential
problem and.immediately initiated actions to properly disposition
the reactor vessel
indications.
Your efforts in,this regard
are
commended.
Nonetheless,
restarting
the plant without thorough evaluation of the pressure
vessel
inservice inspection results
represents
a serious deficiency in your
inservice inspection
program
and
we are issuing the violation enclosed
herewith
as Appendix A.
This violation has
been categorized
by Severity Level in
accordance
with the "General
Statement
of Policy and Procedure
for
NRC
~
Enforcement Actions,"
10 CFR Part 2, Appendix
C (Enforcement Policy).
Your are required to respond to this letter
and in preparing your response
you should follow the instructions
in Appendix A.
