- 1988, and as further described in the Regional Instruction 1440.1/2, dated March 30, 1989.

The narrative sections typically included specific details of the event such as valve identification numbers, model
- numbers, number of operable redundant
- systems, the date of completion of repairs, etc. to provide a good understanding of the event.
Many LERs presented the event information in an organized pattern with separate headings and specific information in each section that led to a clear understanding of the event information.
Previous similar occurrences were properly referenced in the LERs as applicable.
The updated LERs were adequate.

1 AND Pennsylvania Power and Light Company submitted about 63 LERs for Susquehanna Units 1 and 2 during the assessment period from February 1, 1988 to July 31, 1989.
Our review included the following LER numbers:
Unit1'nit 2 88-002 to 88-024 89-001 to 89-019 88-001 to 88-016 89-001 to 89-005 For Unit 1, 17 LERs were caused by equipment failures, 16 by personnel errors and 9 by other causes.
For Unit 2, 11 LERs were caused by equipment failures, 6 by personnel
- errors, and 4 by other causes.
It is noted that equipment fai lures and personnel errors are the primary causes of the LERs.
The LER review followed the general instructions and procedures of NUREG-1022 and the findings and conclusions of this LER review are applicable to both units.
Some of our findings are presented below:
Si nificant 0 eratin Events There were no occurrences in this assessment period that were identified as significant operating events by the AEOD screening and review process.
PNs Issued Durin the Assessment Period Three Preliminary Notifications of Events or Unusual Occurrences were issued for Susquehanna Units 1 and 2 during the assessment period.
They were:
Unit 1 PNO-I-88-025 Turbine control valves closed and automatic scram occurred.
PNO-I-89-008 Unit 2 Control room operators manually scrammed Unit 1 when both Reactor Recirculation Pumps tripped.
PNO-I-88-034 Outlet flow control valve for common fuel pool cooling filter/demineralizer failed open.
The events described in the PNs (PNO-1-88-025 and PNO-1-89-008) were required to be reported as LERs.
The LERs were submitted.
The content of the information in the LERs was in substantial agreement with the PNs.
LER gualit The LERs adequately described the major aspects of each event, including component or system fai lures that contributed to the event and the significant corrective actions taken or planned to prevent recurrence.
