NRC Region I Augmented Inspection Team (AIT) Review of the February 21, 1992, Nine Mile Point Unit 1 Inadvertent Isolation of the Ultimate Heat Sink - Request for-Enforcement Conference The AITreview of the subject event was transmitted separately to you by NRC Inspection Report 50-220/92-80.
Based on the results of this AIT inspection, several apparent violations were identified and are being considered for escalated-enforcement action in accordance with the "General Statement of Policy and Procedure for Enforcement Actions" (Enforcement Policy), 10 CFR Part 2, Appendix C (1992).
The performance of the special test of the "D" screen house gate, which resulted in the inadvertent isolation of the ultimate heat sink, created an unreviewed safety question by placing the plant in an unanalyzed condition.
This is an apparent violation of 10 CFR 50.59.
Also, multiple examples of failure to follow established procedures, inadequate management oversight of worker performance and inadequate communications among work control organizations identified in the AIT inspection report are collectively being considered apparent violations of the Unit 1 Technical Specifications 6.8.1 and 10 CFR 50, Appendix B, Criteria V.
We are concerned about the causes and circumstances that led to this event.
Therefore, an enforcement conference to discuss these apparent violations has been scheduled in the NRC Region I office on April 8, 1992.
The purposes of this conference are to discuss the apparent violations, their causes and safety significance; to provide you the opportunity to point out any errors in the AITinspection report; to provide an opportunity for you to present your short term corrective actions implemented and your long term corrective actions planned with milestones for implementation; and to discuss any other information that will help us determine the appropriate enforcement action in accordance with the Enforcement Policy; 9204010203 920320 PDR ADOCK 05000220 Q
