NRC Region I Combined Inspection Nos. 50-220/91-23 and 50-410/91-23

This refers

to the results of the routine resident

safety inspection

conducted

by Messrs.

W. Schmidt, R. Laura, and R. Temps from October 6 to November 9, 1991, at Nine MilePoint

Units

1 and 2, Scriba, New York. The inspection findings were discussed

with members of

your staff at the exit meeting conducted on November 15, 1991.

This inspection was directed toward areas important to public'health and safety. Areas examined

during the inspection are described in the enclosed NRC Region I inspection report.

Within

these areas,

the inspection consisted of observation of activities in progress,

interviews with

personnel,

and selective examinations of procedures

and representative

records.

Overall, personnel performance at both units was generally good. However, the NRC identified

instances

where personnel

did not adhere

to station procedures

and practices.

At Unit 1,

inspector review of an unresolved item from the previous inspection report identified that the

installation of temporary

gauges

on the core spray

system

was done without an adequate

procedure.

Specifically, the installation was not reviewed for system operability impact, as

required by site administrative procedures.

At Unit 2, an inspector observed

that personnel

failed to perform a proper whole body frisk, in accordance with station radiological procedures,

when exiting a radiologically controlled area.

Based on these findings, these instances in this

report appear to be in violation of NRC requirements,

as specified in the enclosed Notice of

Violation. Further, these cases when combined with other examples of procedural adherence

problems and inattention-to-detail identified in recent NRC reports is of concern to NRC. They

indicate that potentially there is an adverse trend in personnel performance that merits continued

close oversight by utility management.

Based on the results of this inspection, you are required 'to respond to this letter and should

follow the instructions specified in the enclosed Notice when preparing your response.

In your

response,

you should document the specific actions taken and any additional action you plan to

prevent recurrence

of these

types of issues.

Further,

please

include in your response

a

discussion of the actions you have taken or plan to take to address

the inattention-to-detail

concerns and configuration control issues discussed in this inspection report.
