During the period of September

3-16, 1991,

a special Restart

Readiness

Assessment

Team

(RRAT) conducted

an inspection of your corrective actions and readiness

to restart Nine Mile

Point 2 following the August 13, 1991 event.

This event, which resulted in the declaration of

a Site Area Emergency, involved a loss of control room indications and'an automatic reactor trip. The initiating events were the failure of a main transformer and a loss of five non-safety

Uninterruptible Power Supplies (UPS).

The inspection

team

focussed

on the areas of (1) personnel

perform'ance,

(2) maintenance

program effectiveness,

and (3) organizational effectiveness.

The inspection was conducted

to

support a NRC restart decision as a result of the incident inspection activities by the NRC's

Augmented Inspection Team (AIT)and the Incident Investigation Team (IIT). An inspection of

Emergency Preparedness

(EP) activities, which started on August 13, 1991, was also conducted

during this period.

This special

EP inspection

examined

the actions

taken by emergency

response organization personnel in response

to the August 13, 1991 events.

The results of the RRAT inspection, as detailed in Enclosure

1 (Report No. 50-410/91-81), were

discussed with you in the RRAT exit meeting on September

16, 1991.

The team found that the

enhanced

training on UPS operations;

the progress in maintenance program effectiveness;

and

the corrective actions for the problems identified in the area of organizational

effectiveness

during the event were good. The team concluded that the licensee had taken sufficient corrective

actions based on their review for problems identified during the event to support safe restart of

the unit.

The results of the EP inspection as detailed in Enclosure 2 (Report Nos. 50-220/91-19 and 50-

410/91-19), were discussed with Mr. A. Salemi of your staff at the conclusion of the inspection.

The inspector found that the Site Emergency Plan (SEP) was effectively implemented to protect

public health and safety.

As indicated to you in NRC letter dated September

18, 1991, (Nine Mile Point Unit 2 Restart

Authorization and Close-out ofConfirmatory Action Letter 91-13), the NRC concluded, that Nine

Mile Point Unit 2 was ready for restart.

No reply to this letter is required.
