result of the inspection conducted on October 19, 1989 through December 13,
- 1989, and in accordance with NRC Enforcement Policy (10 CFR.Part 2, Appendix C) the following violation was identified:
10 CFR 50.55a(g) requires adherence to Section XI of the American Society of Mechanical Engineers (ASME) Boiler and Pressure Vessel Code for inser-vice testing
( IST) of pumps and valves.
Technical Specification 3.2.6 also requires that IST of pumps and valves be performed in accordance with Section'I of the ASME Boiler and Pressure Vessel Code.
Article IWP-4500, Vibration, of= the ASME Code,Section XI, 1983 Edition including'he Summer 1983
- Addenda, requires in part that "On a
pump coupled to the driver, the. vibratior.
measurement shall be taken on the bearing housing near the coupling".
Contrary to the above, during the performance of Procedure No. Nl-ST-V7 on October 16, 1989,.the IST vibration measurements for the reactor building closed loop cooling system pumps were obtained from an instrument pickup device placed on the inboard mechanical seal housing in lieu of the inboard bearing housing.
This is a Severity Level IV Violation.
