APPENDIX A NOTICE OF VIOLATION Niagara Mohawk Power Corporation
- Syracuse, New York 13212 Docket No. 50-220 As a result of the inspection conducted on January 9-13,
- 1989, and in accordance with the "General Statement of. Policy and Procedure for NRC Enforcement Actions,"
10 CFR Part 2, Appendix C (Enforcement Policy, 1986),
the following violation was identified:
Technical Specification 6. 12, "High Radiation Area", requires, in part, 'that areas accessible to personnel with radiation levels such that a major portion of the body could receive in one hour a
dose greater than 1000 mrem shall be provided with locked doors to prevent unauthorized entry.
In addition, the doors shall remain locked except during periods of access by personnel under an approved RWP.
Contrary.to the above, on December 21,
- 1988, and again on December 23, 1988, the door,to the Flatbed Filter Room, located on the 261'levation of the Old Radwaste
- Building, a designated locked High Radiation Area, was found unlocked.
This is a Severity Level IV violation (Supplement IV).
Pursuant to the provisions of 10 CFR 2.201, Niagara Mohawk Power Corporation is hereby required to submit to this office within thirty days of the date of the letter which transmitted this Notice, a written statement or explanation in reply, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved.
Where good cause is shown, considerations will be given to extending this response time.
