This letter transmits
the results of the routine inspection
conducted
by
Messrs.
W. Cook,
W. Schmidt,
and
R.
Temps of this office on July
7 thru August
24,
1988 at Nine Mile Point Units
1 and 2, Scriba,
A summary of the
findings was presented
to Messrs.
R. Abbott and
R. Randall
and other members of
your staff at the conclusion of the inspection.
The Stop Work Order issued
as
a result of your guality Assur ance
Department'
surveillance of Inservice Inspections (ISI) being conducted at Unit 1, demon-
strates that your Engineering
Department
has not established
adequate
control
and oversight of the program.
Although guality Assurance
was. identifying
problems, effective actions
were not taken before escalation
to
a point where
the Stop Work Order
was necessary.
A Unit
1 Notice of Violation is attached.
This violation shows that further
action is required
by you to prevent
missed Fire Department surveillances
and
improve your administrative controls.
This violation has
been categorized
by
severity level according to 10 CFR Part 2, Appendix C (Enforcement Policy).
You
are required to respond to this violation and in preparing your response
you
should follow the instructions in Appendix A.
In addition, in your response
to this report you are requested
to provide
a
summary of your planned corrective actions regarding:
1) the Inservice In-
spection
Program
Stop Work Order at Unit 1; 2) general
conditions in Unit 1
including poor housekeeping
and cracking of the reactor building wall (section
3. 1); 3) setting of breaker overcurrent trip devices (section
5. l.b.); 4) de-
signating alternates
for the station
managers
without knowledge of a Senior
Reactor Operator (section 12);
and 5)
safety evaluation
reviews
made pursuant
to 10
CFR 50.59 (section
14).
The response
required
by this letter is not subject to the clearance
procedures
of the Office of Management
and Budget
as required
by the Paperwork
Reduction
Act of 1980,
