APPENDIX A NOTICE OF VIOLATION Niagara Mohawk Power, Corporation Nine Mile Point Nuclear Station Docket No. 50-220 License No.

DPR-63 As a result of the inspection conducted on October 1-31, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violation was identified:

Technical Specification 6.8.1 states:

"Written procedures and administrative policies shall be established, implemented, and maintained..."

Administrative Procedure APN-2A, "Conduct of Operations and Composition and Responsibilities of Station or Unit Organization",

Revision 5, Section 4.1.1, dated February 20, 1981, states in part:

"Control Room Log Book shall contain all information pertaining to changing core reactivity during all modes of reactor operation...

Also, entries affecting Station outputs..., unusual conditions...,

etc. will be entered in this log."

Contrary to the above, on October 22, 1981, an entry was not made in the Control Room Log Book concerning the unexplained change in reactor recirculation flow and the accompanying change in reactor power.

This is a Severity Level VI Violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, Niagara Mohawk Power Corporation is hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply including:

(1) the corrective steps which have been taken and the results achieved; (2) the corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved.

Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.

Where good cause is shown, consider-ation will be given to extending your response time.

The response directed by this Notice is not subject to the clearance procedures of the Office of Management and Budget as required by the Paperwork Reduction Act of 1980, PL 96-511.
