SUBJECT: NRC INSPECTION REPORT 50-275I98-007; 50-323/98-007 AND NOTICE OF

VIOLATION

Dear Mr. Rueger:

From February 15 through March 28, 1998, a routine NRC inspection was conducted at your

Diablo Canyon Nuclear Power Plant, Units 1 and 2 reactor facilities. The enclosed report

presents the scope and results of that inspection.

During the 6-week period covered by this inspection period, your conduct of activities at the

Diablo Canyon Nuclear Power Plant facilitywas generally characterized by safety-conscious

operations, sound engineering and maintenance practices, and good radiation protection

support.

Based on the results of this inspection, violations of NRC requirements were identified. The first

violation involved failure to provide a procedure that assured proper stowage of a nonseismic

hoist. This violation was discovered while Unit 2 was conducting midloop operations, and the

hoist was over the operating residual heat removal pump. The second violation involved a

failure to implement refueling procedures

in that the source range monitor high flux alarm was

not restored when the required number of fuel assemblies were placed in the core. The third

violation involved multiple failures to properly implement the clearance procedure.

Clearance

errors have been a previous concern.

The fourth violation involved a failure to translate the

design of the reactor vessel refueling level indicating system into procedures

in that the

shutdown abnormal operating procedures allowed, for several years, the system to be

pressurized to 400 psig when the design pressure of the system was 50 psig. The initial

corrective actions did not address the procedure change process deficiencies that allowed this

to occur.

These violations are cited in the enclosed Notice of Violation (Notice) and the circumstances

surrounding the violations are described in detail in the enclosed report.
