This refers to the routine inspection
conducted
by H. Hiller and F.
Gee during
the period from May 25 through July 6,
1993.
This inspection
examined your
activities
as authorized
by NRC License
Nos.
At the
conclusion of the inspection,
the inspectors
discussed their findings with
members of the
PGKE staff.
Areas
examined during this inspection
are described
in the enclosed
inspection
report.
Within these
areas,
the inspection consisted of selective
examinations of procedures
and representative
records,
interviews with
personnel,
and observations
by the inspectors.
Based
on the results of this inspection, it appears
that certain areas of your
activities were not conducted in full compliance with NRC requirements,
as set
forth in the enclosed
Notice of, Violation (Notice).
These violations involved
(1) two instances
of inadvertent dilution of boron in the reactor
coolant
system, resulting from fai lure to follow or provide adequate
procedures;
and
(2) failure to perform timely reviews of action requests
to determine
whether
a quality e'valuation
was required.
The boron dilution events
represented
a
failure of plant processes
to properly control reactivity due to poor
procedure quality and adherence.
We are also concerned
regarding the length
of time that passed after the initial event before plant management .initiated
corrective action.
In accordance
with 10 CFR 2.790(a),
a copy of this letter, the enclosures,
and
your response will be placed in the
NRC Public Document
Room.
The response
directed
by this letter and the accompanying
Notice are not
subject to the clearance
procedures
of the Office of Management
and Budget as
required
by the Paperwork Reduction Act of 1980,
Pub.
L.96-511.
