inspection conducted

by Mr.

period ot October

17 through December ll, 1989.

This inspection

examined your

activities as authorized

by NRC License

Nos.

DPR-80 and DPR-82.

At the

conclusion of the inspection,

discussions

of our findings were held with you

and other members of your staff.

This inspection involved an examination of multiple problems involving debris

and design deficiencies related to your containment. recirculation

sumps in

Units 1 and 2.

The specific areas

examined during this inspection are

described

in the enclosed

inspection report.

Within these

areas,

the

inspection consisted of selective examinations of procedures

and

representative

records,

interviews with personnel,

and observations

by the

inspectors.

Based on the results of this inspection, it appears

that certain of your

activities were not conducted

in full compliance with NRC requirements.

We

are particularly concerned with the circumstances

surrounding this event

due

in part to the numerous

missed opportunities

which a number of your work

groups

had to fully correct the problems.

Further,

based

on the results of this inspection

and other recent inspections

(Inspection

Reports 89-22,

23 and 25), you should consider. reexamining

and

strengthening

your management

programs

and controls to assure

acceptable

supervisory overview and conduct of activities.

Of particular

concern

from

this inspection report is .the area of design basis

implementation

and

maintenance,

and the conduct of safety-related

surveillances.

As discussed

with Mr. Shiffer on December

8,

1989 by telephone

conversation

with Mr. Marvin Mendonca of my staff, this topic wi 11

be included in the

enforcement

conference

planned for 9:00 a.m.

on December

19,

1989 at the

Region

V offices.

Enforcement related to these matters will be the subject of

separate

correspondence.

In accordance

with 10 CFR 2.790 (a),

a copy of this letter and the enclosure

will be placed in the

NRC Public Document

Room.
