SYSTEMATIC ASSESSMENT
OF
LICENSEE
PERFORMANCE (SALP)
The
NRC Systematic
Assessment
of Licensee
Performance
(SALP) Board has
completed it's evaluation of the performance of your Diablo Canyon facilities
for the period August 1,
1987 through July 31,
1988.
The performance of your
Diablo Canyon facilities was evaluated
in the functional areas
of plant
operations,
radiological controls, maintenance/surveillance,
emergency
preparedness,
security, engineering/technical
support,
safety
assessment/quality
verification, and fire protection.
The criteria
used in
conducting this assessment
and the
SALP Board's evaluation of your performance
in these functional areas
are contained
in the enclosed
SALP report.
Overall, the
SALP Board found your performance
to be acceptable
and directed
toward safe facility operation.
A management
summary of this assessment
is
provided in Section II of the enclosed
report.
Certain strengths,
weaknesses,
and Board recommendations
are provided in Section IV, Performance
Analysis.
In general,
the
SALP Board found that there
has
been progress
in the areas
of
concern identified in previous
SALPs and during this
SALP period.
However,
the progress
observed
has
been
slow..
An example of this slowness
was
demonstrated
in the area of identification, prioritization and reduction of
maintenance
backlog which although identified in the last
SALP period has
moved forward only to the point of improved prioritization, improved
management
reporting schemes,
and very little improvement with regard to
actual
backlog reductions.
Therefore,
we urge you to devote additional attention to thorough
and timely
resolution of the key issues identified in this
SALP evaluation.
Specifically, the
key issues
are (1) to enhance
your efforts to learn from
your own and industry experience,
(2) to improve your activities with regard
to understanding,
maintaining,
and implementing design bases,
and (3) to
develop
a more aggressive self-critical attitude which consistently
resolves
problems in a timely and acceptable
manner.

A related, topic for management
attention,
and further exemplifying the
previously mentioned
slow response
concern,
was demonstrated
soon after the
end of the
SALP period.
Specifically,
on September
1,
1988 both Diablo Canyon
Units tripped.
Unit 2 reactor tripped due to planned surveillance testing of
the seismic trip system.
This reactor trip could have
been avoided, if
appropriate
design
changes
were identified and followed through in a more
aggressive
and timely manner subsequent
to a similar trip on March 5, 1988.
Also, on September
1, 1988, Unit 1 experienced
a reactor trip during an
attempted startup
due to a turbine antimotoring device being inadvertently
valved out.
The closure of the valve had been
noted
and documented
on August
6,
1988 by an engineer
and
made
known to the shift foreman.
The lack of
action in the ensuing
weeks directly caused
the reactor trip.
These incidents
are indicative of an organization that
has not fully established
the proper
attitudes; that is, 1) to identify problems,
2) to take responsibility for
problems,
and 3) to aggressively
pursue the resolution of problems.
It should
be noted that the
SALP Board deliberated
extensively in the engineering
and
technical
support functional area
as to whether
a negative trend was evident,
based
on concerns that arose
during the
SALP period.
The above situations
and
the results of this
SALP clearly indicate that substantive
improvement in the
performance of the Diablo Canyon Units will be limited without a consistent,
ingrained "culture" of aggressive
problem identification,
and timely
resolution
and implementation.
We understand
that you have initiated a study to address
these
problems,
and
look forward to discussing your progress.
A management
meeting to discuss
the
results of the
SALP Board's
assessment
is tentatively scheduled for October
27,
1988, which should afford us
an opportunity to review your progress.
This
date
and the location for the management
meeting will be discussed
with you in
the near future.
In accordance
with Section 2.790 of the NRC's "Rules of Practice,"
Part 2,
Title 10,
Code of Federal
Regulations,
a copy of this letter and the enclosed
SALP report will be placed in the
NRC's Public Document
Room,
as well as
any
comments you may wish to submit to
NRC regarding the content of the
report.
No reply to this letter is required,
however,
should you have
any questions
concerning the
SALP report,
we would be pleased
to discuss
them with you.
