E. Johnston
during the period of March 15,
1987 through
April 25,
1987.
This inspection
examined your activities
as authorized
by
NRC
License
Nos.
DPR-80 and
At the concl.usion of the inspection,
discussions
of their findings were held with Mr.
R.
C. Thornberry,
and other
members of your staff.
Areas
examined during this inspection
are described
in the enclosed
inspection
report.
Within these
areas,
the inspection consisted
of selective
examinations
of procedures
and representative
records,
interviews with
personnel,
and observations
by the inspectors.
Based
on the results of this inspection, it appears
that several
of your
activities were not conducted
in full compliance with NRC requirements,
as set
forth in the Notice of Violation, enclosed
herewith
as Appendix A.
Your
response
to this notice is to be submitted in accordance
with the provisions
of 10 CFR 2.201
as stated in Appendix A, Notice of Violation.
Violation
C in the enclosed
notice is of serious
concern to us in that it
involves operator activities in relation to the use of inappropriate,
unprofessional
wording being labeled
on an annunciator
window.
The fact that
several
plant operators
and their supervisors
were aware of and condoned this
situation is considered significant.
The
NRC is concerned that this instance
represents
a failure of first line supervision,
or higher, to set
a proper
and
professional
tone in the conduct of control
room operations
and that the
enforcement
actions contained in this and other reports
may have resulted
from
such inattention to Control
Room responsibilities.
Violation A in the enclosed
notice raises
the issue of the effectiveness
of
your management
review functions to ensure that appropriate corrective actions
are timely instituted whenever potential safety problems
are brought to your
attention via industry or regulatory
means.
We consider your actions
concerning control of the crosstie
valve in the residual
heat
removal
system
to have
been less
than satisfactory
and urge your immediate attention to.
initiate improvement in this area of management.

A contributing factor in violation
B of the notice is miscommunications
between technicians
and the operating
crew.
The subject of miscommunications
adversely affecting the quality of work or plant operations
was previously
identified in NRC Inspection
Report 50-275/87-10.
In our cover letter dated
March 31, 1987,
PG8E was requested
to provide written" response
describing
appropriate corrective actions
regarding miscommunications.
Your reply to
this request
may be incorporated into your response
to the enclosed
Notice of
Violation.
This
same inspection report identified miscommunications
as
a contributing
factor to a Unit 2 reactor trip experienced
during unit startup.
As described
in the enclosed
Inspection Report 50-323/87-12,
the Licensee
Event Report for
the trip submitted to the
NRC did not identify miscommunication to be
a
contributing factor.
You should take the steps
necessary
to assure yourselves
that this event,
and all future events,
are adequately
assessed
and reported
to the
NRC,
and corrective actions
are taken for all contributing factors to
the event.
In accordance
with 10 CFR 2.790 (a),
a copy of this letter and the enclosures
will be placed in the
NRC Public Document
Room.
