On October 1, 2015, via electronic mail by Ms. Audrey Klett, NRC Project Manager for Turkey Point Units 3 and 4, the Nuclear Regulatory Commission (NRC) Staff requested additional information to complete the review of Relief Request No. 3.

RAI-1

RR-3, Section 4, "Reason for Request," states, in part, that Turkey Point Units 3 & 4 are required to perform VT-3 examination on Class 1, 2 and 3 supports including attachment hardware per American Society of Mechanical Engineers (ASME) Section Xl and that Turkey Point Units 3 and 4 are also required to perform visual examination and testing in accordance with ASME Operations and Maintenance (CM) Code, Subsection ISTD. The licensee's application also states that in order to perform snubber examination and testing (ISTD) and visual VT-3 of snubber attachment hardware (ASME Section XI), FPL has consolidated the Section XI and CM code requirements in the plant Snubber Program. The visual examinations of snubbers is included in Surveillance Procedure No. 0-OSP-1 05-1, "Visual inspection, Removal and Reinstallation of Mechanical Shock Arrestors."

(a) Please verify and confirm that the proposed visual examination criteria of the Snubber Program will be identical to the VT1-3 visual examination per IWA-2213 (IWA-2213(a) thru IWA-2213(g))

(b) The relief request did not mention details about hydraulic snubbers and the proposed update of the procedure is related to mechanical snubbers. Please verify that hydraulic snubbers are not installed at Turkey Point 3 and 4, or hydraulic snubber procedures will be revised to add the VT-3 requirements.

(c) Please provide the ASME Code that is currently being used in the Turkey Point 3 and 4, Surveillance Procedure No. 0-OSP-1 05-1, before consolidation of snubbers examination and its attachments during the fifth 10-year ISI interval.

FPL Response to RAI-1 (a)

FPL included the visual examination of snubber attachment hardware (pin-to-pipe and pin-to-structure) per ASME Section XI along with the visual examination of the snubber assembly (pin-to-pin) when incorporating the criteria of Subsection ISTD into the Snubber Program. Attributes for examinations required by ASME Section XI and the CM Code are included in the procedure 0-OSP-1 05.1, "Visual Inspection, Removal and Reinstallation of Snubbers." FPL confirms that the proposed visual examination criteria of the Snubber Program included in procedure 0-OSP-1 05.1 meet the applicable requirements of IWA-2213(a) through IWA-2213(g) of ASME Section Xl, 2007 Edition with 2008 Addenda.

FPL Response to RAI-1 (b)

Procedure 0-OSP-105.1, "Visual Inspection, Removal and Reinstallation of Snubbers," has been revised and includes VT-3 requirements for the replaced hydraulic snubbers (recently installed in two locations at Turkey Point Nuclear Plant).

ASME OM Code 2004 Edition with 2005 and 2006 Addenda is being used from pin-to-pin inclusively. ASME Section Xl, 2007 Edition, with 2008 Addenda is being used from pin-to-structure and pin-to-piping (associated attachment hardware).

RAI-2

In RR-3, the third bullet of the "Reason for Request" section states that Turkey Point will perform visual examination of 100% of the non-exempt Code Class 1, 2 and 3 snubbers in accordance with the visual inspection frequency required per the ASME OM Code. Please provide the basis to exempt snubbers from the visual examination requirements.

FPL Response to RAI-2:

Non-exempt ASME Code Class 1, 2 and 3 snubber locations will receive a VT-3 examination of the associated component support intervening elements and attachment hardware will be examined in accordance with ASME Section Xl, 2007 Edition with 2008 Addenda in addition to the visual examination of the snubber per ASME OM Code at the frequency prescribed by the ASME OM Code. Non ASME Code Class 1, 2 and 3 snubber locations will receive examination of the snubber pin-to-pin boundary only in accordance with the ASME OM Code. _

RAI-3

Please verify if ASME OM Code Case OMN-1 3 is being used to extend the visual examination interval up to 10 years. While using Code Case OMN-13 for snubbers during the extended interval of 10 years, if the number of unacceptable snubbers (pin-to-pin) exceeds the ISTD-4252-1 limits (these unacceptable snubbers can be found during non-inspection activities such as walkdowns or any other events i.e. water hammer, etc.), what action will be taken and how will these findings align with the examination of snubbers associated attachment hardware (pin-to-pipe and pin-to-structure, excluding integral attachments).

FPL Response to RAI-3:

The Code Case OMN-13 is not currently implemented at Turkey Point Units 3 and 4. Should Turkey Point invoke Code Case OMN-13 in the future, the code case applicability requirements and specifications will be met. This includes counting all visual failures that occur during the inspection interval as stated in Code Case OMN-13, Section 3.2.

Changes to the inspection schedule required by ISTD-4252-1 will result in the same change in schedule for the balance of the support assembly. Findings falling under the jurisdiction of the ASME OM Code have an impact on the inspection interval per ISTD-4252-1. Findings outside the pin-to-pin boundary will be evaluated in accordance with ASME Section Xl and would not be considered a failure when Table ISTD-4252-1 is applied.

RAI-4

IWF-2430(a) states, in part, that component supports examinations performed in accordance with Table IWF-2500-1 that reveal flaws or relevant conditions exceeding the acceptance standards of IWF-3400, and that require corrective measures, shall be extended, during the current outage, to include the component supports immediately adjacent to flawed supports. Please explain how this situation (to include supports adjacent to flawed supports) will be considered, while using the proposed relief request to perform the visual examination of snubbers (pin-to-pin) and associated attachment hardware (pin-to-pipe and pin-to-structure, excluding integral attachments). (Note: The adjacent flawed component supports could be a support or a snubber).

FPL Response to RAI-4:

For flaws or relevant conditions that are discovered within the IWE boundary and that are determined to require corrective measures, additional examinations will be performed in accordance with the requirements of IWF-2430, "Additional Examinations". Flaws or relevant conditions discovered on the snubber, or snubber fasteners, will be evaluated per ASME OM Code requirements.

RAI-5

In RR-3, the second paragraph of Section 5, "Proposed Alternative," states, in part, that program administration and tracking ... ISTD will be applied to the balance of support balance and that the ASME OM Code frequency of examination requires a minimum frequency of 100% per 10 years, which exceeds the requirement for ASME Section Xl. Please explain the statement, "The ASME OM Code frequency of examination requires a minimum frequency of 100% per 10 years which exceeds the requirement for ASME Section XI."

FPL Response to RAI-5:

Currently, Turkey Point Units 3 and 4 do not satisfy the conditions required to extend the snubber examination to a minimum frequency of 100% per 10 years (allowed by Code Case OMN-1 3). Currently, the maximum inspection interval for snubber visual examinations is 48 months. Should Code Case OMN-13 be implemented and the program inspection interval be increased to the 10 year maximum allowable, the complete support assembly would be examined every 10 years. As such, a larger population of examinations is performed of snubbers and their associated attachment hardware, than required by the ASME Section Xl, Table IWF-2500-1, which requires 25% of Class 1, 15% of Class 2, 10% of Class 3, and 100%

of the supports (other than piping supports) over a 10-year interval. Therefore, the ASME OM Code frequency examination exceeds the examination requirements for ASME Section Xl, Table IWF-2500-1.

In the RR-3, Section 5, "Proposed Alternative," FPL references Turkey Point 3 and 4 Procedure 0-aSP-i105-1. In the first paragraph, this procedure is written as Snubber Surveillance Procedure, 0-OSP-1 05-1, "Visual Inspection, Removal and Reinstallation of Mechanical Shock Arrestors." Whereas, in the fifth paragraph, this procedure is written as Turkey Point, Quality Operations Surveillance Procedure No. 0-OSP-I105-1, Please explain the difference between these two descriptions provided for Procedure No. 0-OSP-I105-1, and provide a copy of the Procedure No. 0-OSP-105-1.

FPL Response to RAI-6:

Procedure 0-OSP-105.1 is an Operations Surveillance Procedure. At the time of submittal of RR-3, the title of this procedure was "Visual Inspection, Removal and Reinstallation of Mechanical Shock Arrestors." Since then, a new revision was issued and the title of procedure 0-OSP-105.1 has changed to "Visual Inspection, Removal and Reinstallation of Snubbers." This procedure has been revised to address the inspection requirements for hydraulic snubbers.

As requested, a copy of procedure 0-OSP-105.1 is enclosed.

RAI-7

The contents of the referenced precedence documents listed in the application, Section 7, "Precedents," appear to not be related to the submitted relief request No. 3. Please verify and revise as appropriate.

Section 7 of RR-3 3 has been revised as follows:

The NRC has approved similar requests for alternatives at a number of nuclear facilities, including the following:

  • Relief Request No. 6 was approved for the St; Lucie Nuclear Power Plant, Unit 2 in a letter from the NRC dated June 7, 2015, ADAMS Accession No. ML15097A209.
  • Relief Request No. RR-4-08 was approved for the V. C. Summer Nuclear Station in a Sletter from the NRC dated August 20, 2015, ADAMS Accession No. ML15176A315.

2. Inspections may be performed on painted/coated surfaces when it is determined that such coatings do NOT interfere with interpretation of results.
3. Snubbers which are being removed to facilitate maintenance do NOT require Attachment 5, Snubber Visual Inspection Interval Status Sheet.

1. If the number of unacceptable snubbers is equal to or less than. the number in Category A, the next inspection interval may be twice the previous interval but NOT greater than 48 months.
2. If the number of unacceptable snubbers is equal to or less than the number in Category B but greater than the number in Category A, the next inspection interval shall be the same as the previous interval.
3. If the number of unacceptable snubbers is equal to or greater than the number in Category C, the next inspection interval shall be two-thirds of the previous interval.
4. If the number of unacceptable snubbers is less than the number in Category C, but greater than the number in Category B, the next interval shall be reduced proportionally by interpolation. (That is, the previous interval shall be reduced by a factor that is one-third of the ratio of the difference between the number of unacceptable snubbers found during the previous interval and the number of in Category B to the difference in the Numbers in Categories B and C.)
5. The provisions of Specification 4.0.2 are applicable for all inspection intervals up to and including 48 months.
6. If the snubber population changes,. Table 4.7-2 of the plant technical specifications shall be reviewed for impact of the unacceptable snubber numbers.

RAI-1

RR-3, Section 4, "Reason for Request," states, in part, that Turkey Point Units 3 & 4 are required to perform VT-3 examination on Class 1, 2 and 3 supports including attachment hardware per American Society of Mechanical Engineers (ASME) Section Xl and that Turkey Point Units 3 and 4 are also required to perform visual examination and testing in accordance with ASME Operations and Maintenance (CM) Code, Subsection ISTD. The licensee's application also states that in order to perform snubber examination and testing (ISTD) and visual VT-3 of snubber attachment hardware (ASME Section XI), FPL has consolidated the Section XI and CM code requirements in the plant Snubber Program. The visual examinations of snubbers is included in Surveillance Procedure No. 0-OSP-1 05-1, "Visual inspection, Removal and Reinstallation of Mechanical Shock Arrestors."

(a) Please verify and confirm that the proposed visual examination criteria of the Snubber Program will be identical to the VT1-3 visual examination per IWA-2213 (IWA-2213(a) thru IWA-2213(g))

(b) The relief request did not mention details about hydraulic snubbers and the proposed update of the procedure is related to mechanical snubbers. Please verify that hydraulic snubbers are not installed at Turkey Point 3 and 4, or hydraulic snubber procedures will be revised to add the VT-3 requirements.

(c) Please provide the ASME Code that is currently being used in the Turkey Point 3 and 4, Surveillance Procedure No. 0-OSP-1 05-1, before consolidation of snubbers examination and its attachments during the fifth 10-year ISI interval.

FPL Response to RAI-1 (a)

FPL included the visual examination of snubber attachment hardware (pin-to-pipe and pin-to-structure) per ASME Section XI along with the visual examination of the snubber assembly (pin-to-pin) when incorporating the criteria of Subsection ISTD into the Snubber Program. Attributes for examinations required by ASME Section XI and the CM Code are included in the procedure 0-OSP-1 05.1, "Visual Inspection, Removal and Reinstallation of Snubbers." FPL confirms that the proposed visual examination criteria of the Snubber Program included in procedure 0-OSP-1 05.1 meet the applicable requirements of IWA-2213(a) through IWA-2213(g) of ASME Section Xl, 2007 Edition with 2008 Addenda.

FPL Response to RAI-1 (b)

Procedure 0-OSP-105.1, "Visual Inspection, Removal and Reinstallation of Snubbers," has been revised and includes VT-3 requirements for the replaced hydraulic snubbers (recently installed in two locations at Turkey Point Nuclear Plant).

ASME OM Code 2004 Edition with 2005 and 2006 Addenda is being used from pin-to-pin inclusively. ASME Section Xl, 2007 Edition, with 2008 Addenda is being used from pin-to-structure and pin-to-piping (associated attachment hardware).

RAI-2

In RR-3, the third bullet of the "Reason for Request" section states that Turkey Point will perform visual examination of 100% of the non-exempt Code Class 1, 2 and 3 snubbers in accordance with the visual inspection frequency required per the ASME OM Code. Please provide the basis to exempt snubbers from the visual examination requirements.

FPL Response to RAI-2:

Non-exempt ASME Code Class 1, 2 and 3 snubber locations will receive a VT-3 examination of the associated component support intervening elements and attachment hardware will be examined in accordance with ASME Section Xl, 2007 Edition with 2008 Addenda in addition to the visual examination of the snubber per ASME OM Code at the frequency prescribed by the ASME OM Code. Non ASME Code Class 1, 2 and 3 snubber locations will receive examination of the snubber pin-to-pin boundary only in accordance with the ASME OM Code. _

RAI-3

Please verify if ASME OM Code Case OMN-1 3 is being used to extend the visual examination interval up to 10 years. While using Code Case OMN-13 for snubbers during the extended interval of 10 years, if the number of unacceptable snubbers (pin-to-pin) exceeds the ISTD-4252-1 limits (these unacceptable snubbers can be found during non-inspection activities such as walkdowns or any other events i.e. water hammer, etc.), what action will be taken and how will these findings align with the examination of snubbers associated attachment hardware (pin-to-pipe and pin-to-structure, excluding integral attachments).

FPL Response to RAI-3:

The Code Case OMN-13 is not currently implemented at Turkey Point Units 3 and 4. Should Turkey Point invoke Code Case OMN-13 in the future, the code case applicability requirements and specifications will be met. This includes counting all visual failures that occur during the inspection interval as stated in Code Case OMN-13, Section 3.2.

Changes to the inspection schedule required by ISTD-4252-1 will result in the same change in schedule for the balance of the support assembly. Findings falling under the jurisdiction of the ASME OM Code have an impact on the inspection interval per ISTD-4252-1. Findings outside the pin-to-pin boundary will be evaluated in accordance with ASME Section Xl and would not be considered a failure when Table ISTD-4252-1 is applied.

RAI-4

IWF-2430(a) states, in part, that component supports examinations performed in accordance with Table IWF-2500-1 that reveal flaws or relevant conditions exceeding the acceptance standards of IWF-3400, and that require corrective measures, shall be extended, during the current outage, to include the component supports immediately adjacent to flawed supports. Please explain how this situation (to include supports adjacent to flawed supports) will be considered, while using the proposed relief request to perform the visual examination of snubbers (pin-to-pin) and associated attachment hardware (pin-to-pipe and pin-to-structure, excluding integral attachments). (Note: The adjacent flawed component supports could be a support or a snubber).

FPL Response to RAI-4:

For flaws or relevant conditions that are discovered within the IWE boundary and that are determined to require corrective measures, additional examinations will be performed in accordance with the requirements of IWF-2430, "Additional Examinations". Flaws or relevant conditions discovered on the snubber, or snubber fasteners, will be evaluated per ASME OM Code requirements.

RAI-5

In RR-3, the second paragraph of Section 5, "Proposed Alternative," states, in part, that program administration and tracking ... ISTD will be applied to the balance of support balance and that the ASME OM Code frequency of examination requires a minimum frequency of 100% per 10 years, which exceeds the requirement for ASME Section Xl. Please explain the statement, "The ASME OM Code frequency of examination requires a minimum frequency of 100% per 10 years which exceeds the requirement for ASME Section XI."

FPL Response to RAI-5:

Currently, Turkey Point Units 3 and 4 do not satisfy the conditions required to extend the snubber examination to a minimum frequency of 100% per 10 years (allowed by Code Case OMN-1 3). Currently, the maximum inspection interval for snubber visual examinations is 48 months. Should Code Case OMN-13 be implemented and the program inspection interval be increased to the 10 year maximum allowable, the complete support assembly would be examined every 10 years. As such, a larger population of examinations is performed of snubbers and their associated attachment hardware, than required by the ASME Section Xl, Table IWF-2500-1, which requires 25% of Class 1, 15% of Class 2, 10% of Class 3, and 100%

of the supports (other than piping supports) over a 10-year interval. Therefore, the ASME OM Code frequency examination exceeds the examination requirements for ASME Section Xl, Table IWF-2500-1.

In the RR-3, Section 5, "Proposed Alternative," FPL references Turkey Point 3 and 4 Procedure 0-aSP-i105-1. In the first paragraph, this procedure is written as Snubber Surveillance Procedure, 0-OSP-1 05-1, "Visual Inspection, Removal and Reinstallation of Mechanical Shock Arrestors." Whereas, in the fifth paragraph, this procedure is written as Turkey Point, Quality Operations Surveillance Procedure No. 0-OSP-I105-1, Please explain the difference between these two descriptions provided for Procedure No. 0-OSP-I105-1, and provide a copy of the Procedure No. 0-OSP-105-1.

FPL Response to RAI-6:

Procedure 0-OSP-105.1 is an Operations Surveillance Procedure. At the time of submittal of RR-3, the title of this procedure was "Visual Inspection, Removal and Reinstallation of Mechanical Shock Arrestors." Since then, a new revision was issued and the title of procedure 0-OSP-105.1 has changed to "Visual Inspection, Removal and Reinstallation of Snubbers." This procedure has been revised to address the inspection requirements for hydraulic snubbers.

  • Snubber selection is alternated for accessible and inaccessible snubbers each refueling outage, so all snubbers are visually inspected for each unit in a period not to exceed 48 months.

If it is not possible to resolve the character card at the maximum examination distance required for direct examination, aides for remote examination such as binoculars should be used. Use of visual aids requires demonstration using the VT--3 character card at the maximum examination distance, and most limited lighting, by resolving the .105" characters on the card specified in section 4.1 .5 using the lighting source(s) to be used during examination.

If battery powered lighting is used, demonstration of adequate illumination shall be checked before and after each examination or series of examinations not to exceed 4hrs.

Personnel performing the inspection of the As Left conditions of the snubber must be independent of those personnel performing the reconnection.

1. Plant Technical Specifications, ASME OM Code Subsection ISTD 2004 Edition with 2006 Addenda, and STD-C-01 1, Acceptance Criteria for Mechanical Shock Arrestors (MSA).
2. Snubbers which fail visual inspection and those with indeterminate functionality in Section 4.3 through Section 4.5 shall have Action Requests (AR) initiated.
3. ASME Section Xl, 2007 Edition with 2008 Addenda, IWF-3400 (support elements other than the snubber).

6.2 Functional Criteria

1. Snubbers which appear inoperable as a result of a visual inspection shall be classified as inoperable and may be determined operable for the purpose of establishing the next visual inspection interval provided: [Section 8.1 .2, Developmental 1.C]

A. The affected snubber is functionally tested in the As Found condition and determined operable.

B. The cause of the rejection is clearly established and remedied for that particular snubber and for other snubbers that may be generically susceptible.
