The operator will be told that Unit 1 has just tripped from 100% power, due to seismic activity and that the crew is now implementing EP/1/A/5000/ES-0.1 (Reactor Trip Response). The operator will be directed to monitor the ES-0.1 Foldout Page. The operator will be expected monitor the ES-0.1 Foldout Page, determine that Item #2 is applicable and use Generic Enclosure 20 (CA Suction Source Realignment) to realign the suction of the CA Pumps from the non-safety related to the safety-related source (RN). During the course of this action, the operator will recognize that the RN Supply to the 1 B MDCA Pump cannot be established (Alternate Path), and stop the pump.

JPM B This is a Bank JPM. The operator will be told that Unit 1 is in the process of performing a Unit Start-Up, that Control Bank A has been pulled to 50 steps withdrawn, that Mode 2 has just been declared, that Source Range count rate is approximately 60 CPS, and that SR channel N-31 has failed low, but that SR channel N-32 is still OPERABLE. The operator will also be told that AP/1/A15500/16 Case I (Source Range Malfunctions) has been entered. The operator will be directed to remove the failed SR channel (N-31) from service by performing AP/1/A15500/16 (Malfunction of Nuclear Instrumentation), Case I (Source Range Malfunctions). The operator will be expected to place the Source Range N31 Level Trip Bypass Switch to Bypass, and ensure that Source Range N32 is selected on the NIS Recorder. When the only other channel of SR Nuclear Instrumentation fails high and the reactor does not trip (Alternate Path), the operator will recognize that a reactor trip is required and then manually trip the Reactor.

JPM C This is a Modified Bank JPM. The operator will be told that the plant is at 100% power, and that the IA Cold Leg Accumulator Abnormal Press Alarm has been received. The operator will be directed to respond to the alarm in accordance with the Annunciator Response Procedure. The operator will be expected to address the MCB Annunciator 1AD-9, E-1, select Enclosure 4.3 of OP/1!A/6200/009 to be performed, align N2 to the 1A CLA and raise pressure to greater than 610 psig and less than or equal to 639 psig.

NUREG-1 021, Revision 9

ES-301 Control Room/In-Plant Systems Outline Form ES-301-2 (REV 071814)

JPM D This is a modified Bank JPM. The operator will be told that a Reactor Trip on Lo-Lo S/G Level has occurred due to the loss of both Main Feedwater Pumps, that the CA System will not start, that EP/1IAI5000IFR-H.l (Loss of Secondary Heat Sink) has been implemented, and that Feed and Bleed initiation criteria has been met. The operator will be directed to initiate an NC System Feed and Bleed by performing Steps 22 through 28 of EP/1/A15000/FR-H.1 (Loss of Secondary Heat Sink). When the operator attempts to verify that a high pressure injection flowpath exists, it will be observed that the Safety Injection signal fails to manually actuate (Alternate Path). The operator will be expected to manually start equipment as needed to establish an RCS feed and bleed flowpath such that all NCPs are off, High Pressure Injection flow is established through NI-9/10 and two Pzr PORVs are open.

JPM E This is a New JPM. The operator will be told that with Unit 1 performing a reactor startup, the plant trips due to an electrical malfunction. The crew is currently in EP/1IA/5000/ES-0.1 (Reactor Trip Response). The operator will be directed to perform AP/11A15500/15 (Loss of Vital or Aux Control Power), while the crew continues with this procedure. The operator will be expected to use AP/11A15500/15 (Loss of Vital or Aux Control Power) to determine that normal letdown is not available (Alternate Path), and place excess letdown in service in accordance with EP/1/A/5000/G-1 (Generic Enclosures), Enclosure 2 (Establishing Excess Letdown).

JPM F This is a Modified Bank JPM. The operator will be told that with the plant at power, a Reactor Makeup System failure has resulted automatic Control Rod insertion, that MCB Annunciator 1AD-2, A9, CONTROL ROD BANK LO LIMIT, has alarmed, that AP/1/A/5500/38 (Emergency Boration) has been entered, and that the 18 BA Transfer Pump is OOS. The operator will be directed to initiate Emergency Boration by performing Step 12 of AP/11A15500138 (Emergency Boration). After the operator starts the 1A BA Transfer Pump, Panel 1EXMA will de-energize, also de-energizing the 1A BA Transfer Pump (Alternate Path). The operator will be expected to establish Emergency Boration from the FWST.

JPM G This is a Bank JPM. The operator will be told that a plant transient at Unit 1 has resulted in a reactor trip from power and a Safety Injection actuation, that Safety Injection Termination criteria has been met and that the crew is implementing EP/1/A/5000/ES-1.1 (Safety Injection Termination). The operator will be directed to isolate the NV S/I flowpath in accordance with Step 6 of EP/1/A/5000/ES-1.1 (Safety Injection Termination). While performing this task the operator will recognize that one NV Pump Recirculation Valve cannot be opened (Alternate Path). The operator will be expected to attempt to isolate NV S/I flow per EP/1/A1500/ES-1.1, recognize that this cannot be accomplished and complete Generic Enclosure 18 (Aligning Normal Charging With NV Recirc Path Isolated) to realign the NV system for normal charging, close the SI Injection valves and then maintain a minimum amount of charging flow to avoid overheating the NV Pumps.

RO Admin JPM Summary Ala This is a modified Bank JPM. The operator will be given a set of initial conditions and told that it is desired to insert the Bank D Control Rods about 45 steps. The Operator will be to determine the amount of reactor makeup water needed to obtain the desired Control Rod Height. The operator is expected to calculate a dilution of 2390+/-150 gallons is required to make the specified rod height change.

Al b This is a New JPM. The operator will be told that they are a Licensed Reactor Operator who is currently not routinely standing watch in the Control Room, that their supervisor has directed them to stand watch as needed to maintain their license active, and will be provided with a recent work history. The operator will be directed to complete NSD Form 512-1 to track the maintenance of their license for this quarter, and to identify additional requirements, if any, that must be met this quarter to maintain their license active. The operator will be expected to record the completion of the qualifying watches, on Form 512-1 in accordance with the attached KEY, and identify that 2 additional 12-hour shifts must be stood as either the Ui RO, Ui BOP, U2 RO or U2 BOP by 9/30/14 to maintain the license active.

A2 This is a Bank JPM. The operator will be told that the Plant is in Mode 5 with NCS Temperature at 188°F and stable, and will be provided with current plant data regarding the decay heat removal systems. The operator will be directed to perform Enclosure 13.5 (NC Loop Operability Verification in Mode 4) of PTI1/A14600/003 C (Weekly Surveillance Items), in preparation to transition to Mode 4. The operator will be expected to complete Enclosure 13.5 in accordance with the Attached KEY. This is the same JPM as the SRO Exam.

A3 This is a modified Bank JPM. The operator will be told that GWR Package # 2014013 for Unit 1 Containment Air Release is currently in use to conduct a series of Containment air releases, and that during the first release, conducted using Enclosure 4.2 (Air Release Mode With VQ Flow Monitor Operable) of OP/1/A/6450/017 (Containment Air Addition and Release), the Unit 1 VQ Monitor became inoperable. The operator will be told that the crew stopped the release and continued the air release using Enclosure 4.3 (Air Release Mode with VQ Flow Monitor Inoperable) of OP/1/A16450/017 (Containment Air Addition and Release), and that three previous releases have been made; including the one which was made with the Unit 1 VQ Flow Monitor in operation. Finally, the operator will be provided with the pertinent data for the current (

th) 4 release, and then be directed to calculate the volume released for the current release and to determine the total volume released from the Containment during all releases. The operator will be expected to calculate the volume of air released from the Containment during the final release, and determine the total volume of air released in the series of four releases.

This is the same JPM as the SRO Exam.
