NRC AUGMENTED INSPECTION TEAM REPORT 50-269/96-15,
50-270/96-15, AND 50-287/96-15
Dear Mr. Hampton:
This inspection report documents the Nuclear Regulatory Commission (NRC) response to a
balance of plant heater drain line rupture that occurred on September 24, 1996, at the
Oconee Nuclear Station (ONS) Unit 2. The NRC chartered an Augmented Inspection Team
(AIT) to evaluate the event and ONS response to it. The AIT completed its inspection on
October 8, 1996.
The AIT conducted an independent inspection of the incident and circumstances surrounding
O
it,
monitored the licensee's investigations into the incident and reviewed other related
data/events. The AIT concluded that a water hammer was the cause of the pipe rupture
event and that the immediate cause of the water hammer was the inadequate procedural
controls employed by the station staff during the startup evolution. Several factors
contributed to the inadequate procedural controls including incomplete corrective actions for a
design that was susceptible to a water hammer event, and weak root cause analysis of
previous water hammer events. Further, the lack of engineering and operations management
to plan as priority or initiate actions to minimize water hammers as operational incidents is
considered a factor in causing the event.
The team found that the licensee's response to the incident was in compliance with the
emergency plan, there was no detectable radiation release, and that subsequent actions
taken were appropriate.
In accordance with 10 CFR 2.790 of the NRC's "Rules of Practice," a copy of this letter and
its enclosure will be placed in the NRC Public Document Room.
