INSPECTION REPORT 50-261/89-200 - MOTOR OPERATED VALVE INSPECTION

H. B. ROBINSON STEAM ELECTRIC PLANT, UNIT NO. 2

This letter conveys the results and conclusions of the motor-operated valve

inspection conducted by the Nuclear Regulatory Comni.ssion (NRC)

Office of

Nuclear Reactor Regulation at the H. B. Robinson Steam Electric Plant, Unit

No. 2 during the period of May 22-26, 1989. The inspection team consisted of

regional and headquarters NRC personnel along with one consultant.

The purpose of this inspection was to determine whether the motor-operated valves

in the H. B. Robinson Steam Electric Plant were designed and installed, and are

being maintained, in a manner that would ensure their capability to perform

their intended safety-related functions. During this inspection, technical

reviews were conducted of your engineering, maintenance, training, and operations

as pertinent to our assessment of the motor-operated valves.

As a result of this inspection, two potential enforcement findings and two open

items requiring timely action were identified and were discussed during the exit

meeting on May 26, 1989. The first potential enforcement finding concerned the

inadequate documentation and evaluation of equipment failures. Three examples

of this finding were noted during the inspection, including the failure to docu

ment and evaluate a thermal overload device that was found in a tripped condition

during the last three unit startups.

The second potential enforcement finding concerned four examples of inadequate,

improperly engineered design modifications to the plant. The examples noted

included a case in which the licensee replaced valves with those of a different

type without performing the calculations necessary to ensure adequate motor

actuator sizing.

Subsequently, as a result of IE Bulletin No. 85-03, you

performed these calculations and determined that several safety-related

motor actuators installed in the plant were undersized.

Although your

evaluation supports continuing operation until the next refueling outage, we

understand that you will be replacing the undersized motor actuators.

The two open items identified during the inspection involved your lack of

bases for motor actuator torque switch settings and for thermal overload sizing.

We are particularly concerned that, although the torque switch setting problem

was apparently known well before this inspection was conducted, no preliminary

safety assessment had been performed to ensure the operability of your safety

related motor-operated valves. This fact is particularly troublesome in light

of your response to IE Bulletin No. 85-03, which identified 6 of 11 motor

actuators as being undersized and requiring replacement. On June 6, 1989, a

conference call was held between the NRC staff and Carolina Power & Light (CP&L)

site personnel. During this call, CP&L committed to performing reviews of the

two identified generic concerns within a 90-day period, as discussed in Section 1.3

of the enclosed report.

In addition to the weaknesses noted, the team also noted several positive aspects

of your motor-operated valve program. Specifically, the training of maintenance

personnel, the quality of applicable maintenance procedures, and your review of

motor operated valve generic communication were identified as strengths by the

inspection team.

Any enforcement actions resulting from this inspection will be identified by

Region II in separate correspondence.

In accordance with 10 CFR 2.790(a), a copy of this letter and the enclosure will

be placed in the NRC Public Document Room.
