SUBJECT NRC Bulletin 88-10, Molded Case Circuit Breakers (MCCBs)
Outage Report San Onofre Unit 1, Cycle 11 II. SCOPE This report is provided in accordance with Reporting Requirements specified in NRC Bulletin 88-10 as it pertains to actions taken by Southern California Edison (SCE) for the San Onofre Nuclear Generating Station (SONGS) during the Unit 1 Cycle 11 refueling outage.
This report complies with the submittal commitments stated in the original SCE response for addressing Item 1.c and Item 2 of the Reporting Requirements specified in NRC Bulletin 88-10.
This report completes the actions required by the NRC Bulletin for the Unit 1 Cycle 11 Outage as identified in Non-Conformance Report (NCR) SO1-P-7129.
This report also completes actions for MCCB replacements required due to NRC clarifications made to Position 5 of NRC Bulletin 88-10 based on Supplement 1 to the Bulletin as identified in NCR SO1-P-7432.
III. REVIEW OF PREVIOUS REPORT INFORMATION This report provides information as a result of testing or replacing MCCBs during the Unit 1 Cycle 11 Outage as committed to in the original SCE re sponse. This report does not change information or commitments stated in the initial SCE report or the follow-up report submitted on March 30, 1989, and September 6, 1989, respectively.
SCE has reviewed this outage report for compliance with the clarifications for NRC Bulletin 88-10 as provided in Supplement 1 to Bulletin 88-10 and deter mined that this report is consistent with both the original Bulletin and its Supplement.
IV. COMPLETION OF REQUESTED ACTIONS Listed in Table 1, below, are the installed MCCBs which were replaced or tested, which were subject to the reporting requirements of the Bulletin as identified in NCR SO1-P-7129. The maintenance orders (MOs) or construction work orders (CWOs) under which the MCCBs were originally installed are also identified.

A. Breakers Replaced and Not Tested Of the nine (9) MCCBs listed in Table 1, six (6) were initially scheduled to be replaced rather than tested. The replacements were either traceable MCCBs supplied under 10CFR50 Appendix B, or MCCBs that were tested to SCE's Verifi cation Test Procedure, VTP-E-067. Provided in Table 2, below, is a tabulation of the record ID, Equipment ID, the maintenance work order under which each MCCB was replaced, type of MCCB replacement and the purchase order for which the replacement MCCB was obtained or the construction work order under which the MCCB was tested.

B. Breakers Tested Three (3) installed MCCBs were tested in accordance with VTP-E-067. This procedure complies with Attachment 1 of NRC Bulletin 88-10 for implementing each of the seven (7)
MCCB testing requirements. Of the three (3) MCCBs, two were tested successfully and returned to service. The results of the testing for these MCCBs are listed in Table 3 below.
The MCCB which failed the test was replaced with an MCCB that was successfully tested in accordance with VTP-E-067.
Tabulated below for each MCCB are the Record ID, manufacturer and model number. The procurement chain for each MCCB listed below was provided in Appendix C of SCE's followup report for Bulletin 88-10 which was submitted to the NRC by letter dated September 6, 1989. A maintenance work order is identified for each MCCB installation and a construction work order is identified for each MCCB tested.

MCCB record ID 311 (rated 100 amperes) was replaced during the Unit 1, Cycle 10 Outage with a 90 ampere rated MCCB that was tested to VTP-E-067. SCE Engi neering has evaluated the 90A MCCB installed in position S1-72-120 in accor dance with Proposed Facility Change (PFC) 1-89-014, and concluded that it is adequate to provide proper coordination and protection such that restoration to the original 100A configuration is not required.
The disposition of NCR S01-P-7129 requires that any installed MCCB removed from service is to be returned to the warehouse, the MCCB quarantined, a Warehouse NCR initiated, and a visual inspection performed and documented.
Contrary to these requirements, MCCB Record IDs 296, 311 and 315 were replaced but not returned to the warehouse and cannot be located. However, visual inspections were previously performed on these MCCBs, as documented on VIs #
750, 713, and 714 respectively.
V.
SUPPLEMENT 1. POSITION 5 REPLACEMENTS As part of routine maintenance activities in 1989, six (6) MCCBs that were not within the scope of NRC Bulletin 88-10 were replaced with MCCBs that were not traced to the circuit breaker manufacturer (CBM) but were tested to VTP-E-067.
At that time, SCE considered replacement with non-traceable tested breakers to be within the original guidelines of NRC Bulletin 88-10. Subsequently, these replacements were found not to be in accordance with Position 5, Supplement 1 of NRC Bulletin 88-10 and NCR SO1-P-7432 was initiated to replace the break ers.
The MCCBs that are the subject of NCR S01-P-7432 were replaced by traceable MCCBs during the Unit 1 Cycle 11 outage under the corresponding maintenance work orders (MO) shown in Table 4.
