Gentlemen:

This refers to a meeting held with you, Mr. K. P. Baskin, Mr. C. B. McCarthy,

Jr., and other members of the SCE staff at the San Onofre site on June 27,

1989.

The subjects discussed are summarized in the enclosed report. A copy

of the meeting agenda is enclosed.

At this meeting, we discussed the actions being implemented by SCE to improve

engineering and technical work and the effectiveness of quality oversight

activities in support of San Onofre. In addition, we reviewed SCE efforts in

reorganizing the nuclear engineering and quality assurance organizations, and

relocating these organizations closer to the San Onofre site. We also

discussed the SCE root cause investigation program and implementation of the

Fitness for Duty Rule.*

It appears that SCE has laid proper foundations for improvement in these areas.

However, as discussed during our meeting, we view the lack of Quality Assurance

supervisory presence in the plant spaces to be a significant shortcoming in

assessing plant activities which tends to somewhat offset the effectiveness of

your current initiatives. During the meeting, your newly appointed Quality

Assurance Manager indicated that a primary goal of his near term efforts will

be to emphasize activities which add value to the quality assurance process. I

agree with the concept of "value added" as a means of assessing the effective

ness of quality assurance activities in an organization, and anticipate

improvements over the next few months, particularly in the following areas:

The quality of in-house design activities and the turnover of plant

modifications;

The findings of your nuclear quality oversight activities and the

effective implementation of these findings into program improvements; and

The thoroughness and effectiveness of your root cause investigations and

implementation of effective corrective actions.

We look forward to additional discussion on these matters with SCE during the

remainder of the current SALP period.
