WKM MSIV ROOT CAUSE ANALYSIS AND INSPECTION REPORT (TAC NOS. 68329 AND 68330)

We have reviewed your "Root Cause Analysis for the Guide Rail/Lev-R-Lock Interaction Problem for MSIVs" dated June 19, 1988, your "Inspection Results of SONGS - 3 MSIV -

8205" dated July 7, 1988, and your revised "Inspection Results of SONGS - 3 MSIV - 8205" dated August 24, 1988. As we stated in our Request for Additional Information dated August 18, 1988, we do not find the continued operation of San Onofre Units 2 and 3 to constitute a significant safety hazard, however, the incorrect conclusions drawn from your boroscopic examination cast doubt upon the adequacy of your proposed long term actions.

Therefore, we took particular note of your boroscopic inspection improvements described on pages 16 and 17 of your revised inspection report and the flow orifice installation described on pages 17 and 18 of the same report. While these actions should enhance the operation and examination of the MSIVs, we are not yet convinced that they are sufficient to preclude disassembly of MSIVs for inspection. To provide additional confidence that a boroscopic examination alone is sufficient, we request that you notify us of the date of the next boroscopic examination as soon as it becomes available so that we may observe the operation. In the meantime, we withhold comment on the acceptability of your recommended inspection plan.
