NRC Exam Question Analysis The following questions were missed by > 50% of the students on the Reactor Operator section of the ILT 2013 class NRC Written exam:

  • Question 5 (Missed by 60% of class)
  • Question 51 (Missed by 50% of class)
  • Question 52 (Missed by 50% of class)
  • Question 55 (Missed by 70% of class)
  • Question 61 (Missed by 100% of class)
  • Question 72 (Missed by 50% of class)
  • All questions were validated. All questions (Except Question 61) are deemed as knowledge deficiencies. Review of question, correct answer and why distractors were wrong was completed as part of the examination review. Training material review indicated material presented allowed for correct answer to be derived.
  • Question 61, even though covered by an objective in the training material presented to the students, its deemed a training issue since the entire population missed the question. A majority (80%) missed the question due to not identifying the correct technical specification required boron concentration of the question. A course enhancement (# XXXXXX) was entered into the Callaway Plant Training Application with the following direction: Ensure Technical Specification 3.9.3, Nuclear Instrumentation, is covered to ensure students understand the requirement to prevent reducing RCS coolant boron concentration below the limit of LCO 3.9.1.

This concept was missed by 100% of the students on the ILT 2013 NRC Written examination.

The following questions were missed by > 50% of the students on the Senior Reactor Operator section of the ILT 2013 class NRC Written exam:

  • Question 81 (Missed by 50% of SRO students)
  • Question 83 (Missed by 67% of SRO students)
  • Question 91 (Missed by 50% of SRO students)
  • All questions were validated (See discussion of Question 83 below). All questions (Except Question 83) are deemed as knowledge deficiencies. Review of question, correct answer and why distractors were wrong was completed as part of the examination review. Training material review indicated material presented allowed for correct answer to be derived.
  • Question 83 - All students that missed this question selected answer B:

o Question 83 could have 2 possible answers depending on how information provided in the stem of the question is interpreted by the students. The question is:

The plant is operating at 100% when the Shift Manager enters OTO-ZZ-00001, Control Room Inaccessibility, due to heat and smoke in the Control Room.

The Control Room Supervisor (CRS) will direct the Control Room Operators to

  • It is agreed that the answer to part (1) of the question is Trip all Reactor Coolant Pumps which eliminates answers A and C.
  • It is agreed that if the students assume there is a Fire in the MCR then the CRS would go to the Auxiliary Feedwater Pump Rooms, so answer D is correct as validated.
  • However, if the students do NOT assume there is a Fire in the MCR then OTO-ZZ-00001 step 4 RNO would direct the CRS and Balance of Plant Operator to perform Attachment G. This attachment sends both of these operators to the Auxiliary Shutdown Panel. This would make answer B correct.

o Justification for assuming there is no fire:

When determining EALs, the students are trained that smoke without indication of a fire is classified as a Toxic gas which is separate from the classification of Fire or Explosion.

o Without the stem explicitly stating the MCR was being evacuated due to a FIRE the students that missed the question assumed there was no fire.
