Control Bank D has been inadvertently withdrawn past 230 steps D Bank step counters currently read 234 steps.
The crew has entered 3-ONOP-028, Reactor Control System Malfunctions, and completed the immediate actions.
Initiating Cue:
The US has directed you to restore the control rod system to its normal configuration by performing Section 5.5 of 3-ONOP-028, Reactor Control System Malfunctions.
The SM has authorized the return of the Control Rods to AUTO on completion of the procedure.

The US has directed you to flush the seat of PCV-3-456 by performing section 5.4 of 3-NOP-041.2, Pressurizer Operation.
A7/2, PZR PORV HI TEMP, is an expected alarm during the seat flushing evolution.

The operator will flush the seat of PCV-3-456 in accordance with Step 5.4.2 of 3-NOP-04i.02. During the process the PORV will fail OPEN.
The operator will respond to annunciator A7/2 in accordance with 3-ARP-097.CR.A, enter 3-ONOP-041.5 and attempt to close the failed PORV. When the PORV fails to close the operator will attempt to close the PORV Block Valve (Alternate Path). When it is determined that Pzr pressure cannot be maintained greater than 2000 psig, the operator will trip the reactor and perform the immediate actions of 3-EOP-E-0 based on the 3-ONOP-041.5 Foldout Page criteria.

The following guidelines are required to be followed to reduce the probability of EDG overload conditions without Main Generator Lockout protection:
If the 3A 4KV bus is NOT powered via the auxiliary transformer (3AA02 open), special attention is required to be given to the 3A EDG operating parameters during parallel operation to the system and the EDG is required to be tripped upon indication of impending overload.
CAUTIONS If the 3A 4KV bus is powered via the auxiliary transformer (3AA02 closed), no special precautions are required because protection is provided by the Main Generator Lockout.
Starting any of the following pumps may cause an EDG paralleled to the affected 4160V bus to trip and may cause damage to the EDG:
- 1) Reactor Coolant Pump
- 2) Condensate Pump
- 3) Steam Generator Feed Pump
- 4) Heater Drain Pump

Task Standard:
The operator will locally trip the A AFW Pump. Upon recognizing that the C AFW Pump is not running, the operator will reset and start the C AFW Pump, and verify that the B AFW Pump is running.

(Step 14) REQUEST RCO observe R-i8 to ensure both the following:

EXAMINER NOTE Valve 1279, MONITOR TANK PUMP RECIRC TO MONITOR TANK A STOP, throttled % turn OPEN, should prevent Recycle Monitor Tank NOTE Pump cavitation, however, if the pump exhibits cavitation characteristics, 1279 should be throttled while maintaining a recirculation flowpath.
(Step 16) MONITOR operating Recycle Monitor Tank Pump for STEP 10 cavitation SAT IF cavitation is evident, THEN UNSAT The operator listens to pump operation/observes discharge pressure Standard:
for signs of cavitation of the RMT Pump B.
The operator reads the conditional step, and continues.
The RMT Pump B discharge pressure is steady at 70 psig.
Cue The RMT Pump B sounds normal.

Appendix C Form ES-C-i JPM BRIEFING SHEET INITIAL CONDITIONS:
A Liquid Release Permit for Recycle Monitor Tank A has been approved by the Shift Manager.
The Tank Recirculation and Verification Sheet has been approved by the Shift Manager.
The appropriate procedure for performing a liquid release has been obtained verified current.
All release prerequisites have been satisfied.
The Recycle Monitor Tank A Pre-Release Alignment has been verified.
The US directs the use of Waste Monitor Tank Pump B R-18 is operable.
INITIATING CUE:
The US has directed you to perform a Liquid Release of the Recycle Monitor Tank A starting at step 7 of Attachment 1 of O-NOP-06i.iiA, Controlled Liquid Release from Recycle Monitor Tank A.
NOTE: No plant equipment should be operated during the performance of this JPM. All actions must be SIMULATED.
