  • The OAC reported to work feeling ill and his illness has now become so severe that he can no longer stand watch You have been directed to assume the OAC position.

The CRS has authorized the watch relief without a face-to-face turnover.

Initiating Cue: Continue the reactor startup to the POAH lAW GP-004, Step 46.

A second operator will acknowledge, silence, and report any alarms not related to the reactor startup.

Time Critical Task: N/A Validation Time: 15 minutes CRITICAL STEP JUSTIFICATION SUR above 1 DPM is not allowed by procedure and could potentially Step 2 place the Reactor in an unsafe condition where an automatic Reactor trip is generated.

Stabilizing power at the POAH (1% 3%) is required by procedure and Step 4 stabilization at a power level less than 5% is below a mode change from Mode 2to Model.

Determining Reactor Trip criteria is required to understand that the Step 5 Reactor is no longer in a safe controlled condition and is critical to determine that the system is not functioning properly.

  • Place reactivity data sheet for IC-i2 on plant status board (this will be used for obtaining boron concentrations for the RCS and BAT)
  • May have to update rods
  • Place Restricted Access signs on swing gates
  • Go to RUN and wait 10 seconds then silence and acknowledge alarms.

GO TO FREEZE and inform the lead examiner the Simulator is ready. DO NOT GO TO RUN until directed by the lead examiner. (The examiner has provided to the candidate with initial conditions and the initiating cues prior to placing the simulator in RUN.)

To recreate the IC setup for this JPM:

  • Reactor trip low Pressurizer pressure approaching setpoint (1960 psig rate sensitive)

GP-004 Precautions and Limitations #19, #20 and #21:

19. If Tavg goes below 551°F and can not be restored to greater than 551°F, within 15 minutes, and in a controlled manner, the Reactor should be tripped. Excessive reactivity additions to restore Tavg could result in a rapid power increase.
20. If Tavg goes 10°F higher or lower than Tref and the trend shows no sign of turning the reactor should be tripped. A loss of temperature control could lead to an ESF actuation.
21. If a steam leak occurs, and reactor power is approaching 10%, the reactor should be tripped. This preemptive action is desired since the reactor should automatically trip, on TURBINE TRIP/REACTOR TRIP, at 10% power.

  • Obtain Safety Key Locker key from the CRSs desk
  • Identify which key is required based on the list in key locker area

When the need for a key is determined ask the candidate how to obtain the proper key and note the response. (See details on previous page for obtaining a cabinet door key). Then provide the candidate with the cabinet key.
